Job overview
We have an exciting opportunity for band 5 Dietitian to join our Merton Community Nutrition and Dietetic team. You will be an enthusiastic, self-motivated individual who has an interest in developing skills in gastroenterology and nutrition support in a proactive department. The role supports patients with long term chronic conditions and dietary education. Those at risk of malnutrition, adopting Food First guidelines and appropriate prescribing of Oral Nutritional Supplements. You will be providing a Dietetic Service in the London Borough of Merton; working closely as part of an MDT service alongside Neuro Rehabilitation Team. As well as the wider Nutrition Support and Gastro workstream within CLCH.
You will be responsible for:
· Your own varied caseload of domiciliary patients
· A caseload of care home and nursing home residents requiring oral nutrition support and in some instances enteral feeding
· Running outpatient clinics
The dietetic department currently undertakes student training for 1, 2 and 3 placements. You will be actively involved in supporting students. CLCH has a robust CPD programme, and we are committed to providing excellent line management, supervision / support, and specialist training. In order to support you in this role, you would receive training and supervision tailored to your needs.
Main duties of the job
· To provide, under the supervision of your supervisor, a clinical dietetic service to adults in the community of Merton.
· This will mainly comprise of assessing and reviewing a caseload of patients requiring oral nutrition support in their own home and in care homes.
· Managing a small caseload of patients requiring Home Enteral Feeding in care homes.
Carrying out regular virtual clinics within the community setting for patients with long-term conditions including gastro, neurological and cardiovascular diseases

Qualifications
Essential criteria
- Degree in Nutrition and Dietetics.
- HCPC state registration.
Desirable criteria
- Relevant MSc or post registration qualification.
- Member of BDA.
- Member of a special interest group of the BDA.
Experience
Essential criteria
- Experience in managing patients requiring nutrition support for both oral and enteral nutrition.
- Experience of audit.
- Understanding of dietetic service within a busy community team.
- Understanding of the importance of effective multidisciplinary working.
Desirable criteria
- Experience of working at a band 5 level.
- Experience working in Community Dietetics, Nursing and residential Homes.
- Experience in training/teaching healthcare professionals.
- Experience of using behaviour change techniques in clinical practice.
Skills
Essential criteria
- • Good communication and report writing skills, in both verbal and written format.
- • Basic IT skills – Microsoft Word, Powerpoint, MS, teams, Excel.
Desirable criteria
- Knowledge of long-term neuro conditions and chronic diseases with associated national policies and guidelines
- Knowledge of functional gut disorders and associated policies and dietetic guidelines
Key Attributes
Essential criteria
- Ability to travel too and around the London Borough of Merton to visit patients in their own homes and care homes using public transport or car
Desirable criteria
- Experience of working with EMIS.
- Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)
