The service mission is to provide a high quality service for people with severe and enduring mental health needs by working in active partnership with service users, carers and with local voluntary and community groups, in addition to statutory services within the Primary Care Network (PCN) Structure in Newham. The 10 Newham PCNs are grouped in 5 Mental Health PCNs, with 5 aligned CIMH Teams, managed by a North and South sector manager. CIMHS is an emergent service, integrating a range of existing offers, including ELFT’s Enhanced Primary Care (EPC) and Assessment and Brief Treatment (ABT), with new provision funded via NHSE early implementer resource, including significant 3rd Sector provision, Peer Support and existing Social Care provision.

-The post holder will work as an autonomous and accountable practitioner within an integrated team.

- To provide clinical and management supervision for  band 6  and other staff as agreed

- To provide a consistently high standard of mental health care to people in Newham; to assess patients based on sound theory, principles and practice within a nursing context in line with the CIMHS operational guidelines.

- To liaise with other care providers in Newham in order to facilitate a comprehensive service for individual service users.

- To promote and maintain a specific focus and interest in the care of people with mental health needs.

- To liaise and work closely with the team’s attached ward in order to promote a seamless service between services.

Swydd ddisgrifiad a phrif gyfrifoldebau manwl

To assess clients who are referred or refer themselves within a health and psycho-social framework, and make clinical decisions as required, involving the wider MDT as needed.

- The post holder will link closely with the Primary Care clinics and the catchment area secondary care MH teams, attending referrals meetings to expedite primary care referrals and provide feedback as required.

-The post holder will offer consultation, advice and clinical supervision to other members of the clinical team regarding Primary Care services. 

- To understand the social factors which are related to mental health needs and to be aware that respect must be given to customs, values  and spiritual beliefs of all service users, particularly those from ethnic communities.

-The post holder will provide specialist advice, consultation and clinical supervision to allocated staff working in primary care. 

- To develop and maintain a high standard of practice based on comprehensive assessment and employing a range of therapeutic approaches.

- To all ensure all service users have a care plan which is agreed, implemented, monitored and reviewed as required.

- To work jointly with colleagues from within the team or within the statutory and non-statutory agencies, either through direct client contact or in a clinical consultative capacity.

- To keep accurate up-to-date records of all contacts with service users and professionals involved in provision of services.

 

-The post holder will work with the GP practices and Consultants to plan and co-ordinate the transfer of patients back to primary care for depot medication in line CMH Transformation framework.  This transfer will be based on a systematic analysis of this group of patient’s needs which will be undertaken by the post holder.

 

- To ensure that all changes of circumstances relating to the service users are communicated to the appropriate persons/departments, including family/carers where appropriate.

- To participate in the triage, assessment and review of patients referred to the service and to assist in providing a crisis response. The post holder will provide clinical advice and interventions to GP practices participating in the Depot Service, in order to support the provision of care for those patients.

- To liaise between PCN teams, G.P Practices, and secondary care MH services offering advice, support and education on mental health issues.

- To prioritise and attend all team meetings.

- To receive regular appropriate management supervision as agreed with the CIMHS manager.

- To be a representative of the CIMHS in meetings/forums as agreed with the CIMHS Manager.

- To engage in reflective and managerial supervision with the CIMHS manager and to ensure that such sessions are maintained.

-To provide clinical and management supervision for band 6 and other staff as agreed.

  • Relevant -RMN/ Social work / OT / Qualification.
  • Experience of Clinical Leadership, Well-developed and suitable experience of working as a qualified in adult community mental health services. Experience of providing mentoring and supervision to junior staff and students on placement.
  • Experience of providing clinical leadership/ management at band 6 level or above.

  • Evidence of regular up-dating in practice Leadership training
  • Experience of leading and managing change
