West London NHS Trust  provides a full range of mental health, community  and physical healthcare services for children, adults and older people living in the London boroughs of Ealing, Hammersmith & Fulham and Hounslow.

We also provide some specialist services that are commissioned regionally, such as our medium secure services; and nationally, such as the Cassel Hospital for people with complex and severe personality disorder and our high secure services at Broadmoor Hospital.

An exciting opportunity has arisen for an enthusiastic pharmacist to join a friendly, progressive medicines management service that serves our community in Ealing. The role is to Lead the medicines management service for the Rapid response virtual ward. This supports admissions avoidance and early discharge from acute settings. You will also lead medicines management support for Ealing Community health services that includes multiple community services such as MSK, diabetes service, district nursing, special schools, children with learning disabilities, SALT, physiotherapy, dietetics, podiatry and TVN. 

The successful candidate should be a highly motivated team player with good communication skills and a keen interest in treatment of long term conditions and medicines optimisation. Experience of working with medical patients is preferred.

Prif ddyletswyddau'r swydd

The post holder will : 

Lead on developing the medicines management service for the Rapid response virtual ward, supporting the multidisciplinary team with all medicines related issues and optimising medicines use for patients under the care of the service.

Be responsible for providing medicines management support and medicines information to community based teams including: MSK, diabetes service, district nursing, special schools, children with learning disabilities, SALT, physiotherapy, dietetics, podiatry and TVN

Be responsible for managing and delivering comprehensive prescribing advice and information for clinicians, patients and carers.

Be responsible for ensuring the safe, clinically effective and cost efficient use of medicines. 

To initiate, supervise and undertake medicine audits (where required) on behalf of the pharmacy team.

Provide appropriate training and education for nursing and medical staff with regards to the use of medicines used in integrated care and other therapeutic areas

Collate activity and outcome data for respective areas

To actively develop, monitor and integrate policies/guidelines and procedures covering all aspects of pharmaceutical care and medicines management within the service.

The applicant will be required to take part in the out of hours on-call service as well as the weekend morning service on a rota basis. There are additional payments surplus to the main salary for these shifts.

Experience
