The post holder will be a key member of the Bed management and complex discharge team supporting the ward based and complex discharge nursing team to ensure an organised and smooth referral process and transition/ discharge for all complex patients.

 

The role will ensure that the ward based and complex discharge nursing team are supported administratively with all aspects of their work. You will have a key role in liaison with internal and external teams.

  • To assist the complex discharge team in all administrative tasks
  • To co-ordinate complex discharge MDT’s ensuring timely invitations and completing written minutes of the meeting outcome
  • To liaise with external agencies to ensure correct referral details
  • To ensure the complex discharge teams databases are maintained 
  • Work alongside the senior sister or lead nurse to complete complex discharge audits and have an active role in service development and improvement projects.
  • To work in a stressful and unpredictable work environment whilst managing and prioritising the workload. 
  • In the absence of the lead administrator take minutes for the complex case meeting, utilising NoteON, ICS and ensuring accurate records.
  • To ensure good working relationships and work closely with clinicians, ward and specialist nurses, schedulers, therapists, within the RNOH to ensure that complex discharges are actively managed through the patient pathway.

·         Contribute to effective information management within the team.

 

·         Establish, maintain, prioritise and review appropriate and effective e-mail groupings and contacts for local ICB’s to enable senior nursing colleagues to escalate and complex discharge delays or concerns.

 

·                  To establish and maintain appropriate records and filing systems facilitating easy retrieval and access of information.

 

·         Manage the email system for the clinical team, as required and respond to mail as appropriate.

 

·         Develop and maintain databases as required by the role

 

·         Maintain administrative and information resources.

 

·         Maintain a thorough knowledge of and ensure adherence to the full range of policies / procedures and guidelines relevant to the unit 

 

·         Analyse and report on data and monitor the processing of data and information, as required.

 

 

2.3   Finance

 

·         Contribute to the financial delivery of the department objectives

 

 

2.4 Organisation and Planning:

 

·         To maintain up to date knowledge of all targets and processes relating to complex discharge and to be a point of reference within the department for members of the multi-disciplinary team

 

·         To work closely with members of the clinical and non-clinical teams across the Trust in developing and refining policies and procedures to ensure seamless management of patients across the department.

 

·         To liaise with colleagues, other departments or organisations to support the patients

2.5 Service Development:

·         Propose changes to and make recommendations for more effective service delivery.

 

·         Contribute to the review and development of existing project information management systems

 

·         Support implementation of projects and service initiatives through timely and relevant information analysis and administrative support, in accordance with the agreed priorities of the team. 

 

·         Participate in relevant internal working groups/projects to provide information, advice and support and to maintain data collection systems for its effective use by the team

 

 

2.6 General

 

·         The post holder will participate in a personal development plan and attend all mandatory training, as appropriate.

 

·         To comply with all Trust policies and both legal and statutory requirements

 

·         To comply with the Trust Equal Opportunities policy and treat staff, patients, colleagues and potential employees with dignity and respect at all times.

 

·         To take personal responsibility for promoting a safe environment for staff and patients by identifying  areas of risk and following the Incident, Serious Incidents and Near Misses reporting policy and procedure.

 

·         To comply with the Trust policies for personal and patient safety and for the prevention of healthcare associated infection (HCAI); this includes a requirement for rigorous and consistent compliance with Trust policies for hand hygiene, use of protective equipment and safe disposal of sharps.

 

·         In accordance with the Trusts responsibilities under the civil Contingencies Act 2004 to undertake work and alternative duties as reasonably directed at variable locations in the event of and for the duration of a significant internal incident, major incident or pandemic

 

·         To be aware of and adhere to all Trust policies and procedures including Health and Safety at Work Act and the Data Protection Act

 

·         To maintain confidentiality at all times
