Community Early Intervention Practitioner

We have been working together in a Partnership between Barnet, Enfield and Haringey Mental Health NHS Trust and Camden and Islington NHS Foundation Trust for some time, with a single Chair and Chief Executive since 2021 and single Executive Team since 2022. We launched our North London Mental Health Partnership Strategy in May 2023 and in July 2023 our Boards agreed that coming together to create one new Trust is best for our service users, our staff and our local communities. Subject to final approvals, we will be formally established as one new Trust on 1 October 2024.

The Haringey Early Intervention in Psychosis Team is seeking to recruit a Band 6 or 5 Registered Mental Health Nurse, Occupational Therapist or Social Worker to join our fully substantive team as a Community Early Intervention Practitioner. We invite applicants as Band 5's who will be supported with our band 5-6 development program. 2 posts have become vacant following the career development.
Our team provide a range of specialist mental health interventions to people aged 18-65, living in Haringey and are presenting a first episode of psychosis. We have a diverse, and well-supported multidisciplinary team including Nurses, Doctors, Social Workers, Systemic Family Therapists, Psychologists, Cognitive Analytic Therapists, Dual Diagnosis Workers a Peer Support Worker and an IPS employment worker. Both our IPS worker and DDN are embedded in the team.
We highly value social inclusion and co production. We and have established good working relationships with other organisations, individuals, carers and service users.
We a thriving garden project and will be laying the flooring for our bike project next week. We have a pet therapy dog trainee post.
We have commitment to CPD support, clinical fieldwork education, evidence-based practice supported by internal and external training. We currently have 5 trainees in the team.

Job Summary:
To undertake the role of care co-ordinator in the assessment, development and delivery of support to people experiencing first episode psychosis. Working as part of the multi-disciplinary team you will work with individuals and their families or carers providing support, education, family work and evidence based interventions aimed towards promoting recovery. You will work with people aged between 18 and 64 years in a service which aims to work with other agencies including primary care to raise awareness of psychosis and to reduce the length of time people remain untreated.
To supervise, instruct and teach junior and unqualified staff and students
To undertake duties designed to extend clinical and managerial expertise as delegated by the manager when required. To provide a full care management service to clients and their carers.

Main Tasks and Responsibilities:
- Carry overall responsibility for the management of a caseload acting as a care co-ordinator /care manager, as defined by the Care Programme Approach and other government and local directives. Ensure care is carried out effectively within a multidisciplinary framework and in collaboration with other agencies. Maintain contact with clients on their caseload during periods of admission, including regular attendance on the ward. To take lead in the planning and implementation of the client’s re-integration back into the community.
- Provide patients and their carers with a service that is culturally sensitive and gives due consideration to any special needs.
Provide therapeutic interventions by means of assessment and on-going treatment in various settings. Design care programmes in line with risk assessment and psychosocial needs. Discharge clients in line with the Early Intervention policy and procedures
- Maintain clear, comprehensive and contemporaneous written clinical records on all clients in accordance with Trust policy, utilising I.T. skills and knowledge to access and input clinical information and data to various electronic system, mainly RIO
- Provide formal, on-going clinical supervision and caseload management supervision to junior staff and Support Workers who deal with delegated cases. This includes carrying overall responsibility for their caseloads
- Undertake formal and informal teaching of qualified and unqualified staff, including students
- Initiate and/or collaborate in clinical developments based on appropriate research evidence. Set, monitor and ensure maintenance of standards of care.
- Promote health and provide highly specialist advice / education regarding mental health to professionals, patients, their carers, other agencies and the public in general.
- Receive regular, on-going clinical and case management supervision; attend staff development and training as required. The post holder will be subject to periodic appraisal for their work performance (Individual Performance Review) and objectives will be set on an annual basis between the post holder and their manager
- To prepare and provide social circumstances report and mental health tribunal report as required.
- To prepare and provide Community Care Assessments, Carers Assessment, Financial assessments. To negotiate buying in Services as required in meeting client’s needs. E.g. Direct payments, Home Care, Housing.
- To provide a direct link / liaison to GP practices as a mental health specialist advisor.
- To assist clients with Welfare Rights / Benefits / Freedom Pass applications as necessary.
- Promote Health and Safety advice at work and in the Community.
- To be highly skilled in working with potential aggressive behaviour of clients, relatives and carers in the community.
- To have specialist skills and knowledge required to work independently in highly unpleasant/potentially hazardous conditions and unpredictable environments.
- To participate with the recruitment process of new staff.
- To be involved in the Disciplinary Action Process of junior staff.
