Job overview

We have an exciting opportunity within our team for the Acute Frailty service at Hillingdon.
We are looking for a part-time (0.5 wte) qualified ACP with Frailty experience. 
The Post-holder will work within our Consultant led multi-disciplinary team in the dedicated Frailty SDEC unit.
As an acute leader in acute frailty service provision across the sector the unit supports direct community and LAS referrals as well as supporting flow through the Hillingdon emergency department. The post-holder will join a team of ACP's and trainee ACP's working with specialist nurses and therapists in the Frailty Unit providing clinical assessment, diagnostic skills and comprehensive geriatric assessment to frail adults presenting acutely, with an aim of avoiding unnecessary admission to hospital.

Main duties of the job

This is an exciting chance to be a part of our new multidisciplinary Frailty Assessment Unit (FAU). Our new unit provides an alternative pathway for older adults with frailty who present to hospital in an emergency. It is co-located with our Emergency Department. The unit accepts direct referrals from the community, London Ambulance Service and our Emergency Department.

Essential criteria
  • Professional registration with NMC / HCPC / GPC
  • Prescribing Qualification or meet criteria to undertake within 6 months of appointment
  • Management and Leadership qualification
  • Extensive demonstrable experience in Frailty
  • Experience of implementing evidence-based practise
  • Excellent clinical and practical skills as an autonomous practitioner
  • Ability to communicate with staff at all levels
  • Ability to recognise and analyse complex situations and take appropriate action
  • High level of personal motivation
Desirable criteria
  • Information technology skills
