The Respiratory Specialist Nurse/physiotherapist will work within a multidisciplinary respiratory team to deliver the Respiratory Service across CLCH within the London Borough of Wandsworth.
To take a lead role in the assessment and treatment of patients within the speciality who have highly complex physical, psychological and social needs and work as an autonomous practitioner.
Work with the Respiratory team in assessing, planning, co-ordinating, delivering and evaluating care for patients receiving domiciliary and clinic respiratory disease management including exacerbation management, care plans, self-management, pulmonary rehabilitation, admission avoidance, home oxygen and stable disease management at home.
The post holder will work jointly within the Respiratory Team to plan, co-ordinate, implement and evaluate the service provided to respiratory patients.
Main duties of the job
· To use specialist respiratory knowledge and skills to safely identify patients with an acute respiratory exacerbation that are suitable for admission avoidance and self-management care plans using the NICE guidance for safe care planning and home management.
· To undertake comprehensive assessments of patients with suspected respiratory disease, including history taking, physical examination, spirometry, formulating individualised management and treatment plans using clinical reasoning.
· To undertake comprehensive assessments of complex respiratory patients with co-morbidities, including physical examination, and to develop treatment plans for patients using investigative and analytical skills, and to formulate individualised management and treatment plans using clinical reasoning.
· To work with the team in developing and implementing specialist care programmes, including home oxygen therapy, pulmonary rehabilitation, psychological interventions and medication reviews, adhering to evidence based guidelines.

Detailed job description and main responsibilities
· To maintain accurate, comprehensive documentation, ensuring patients care plans / documentation is up dated to reflect changes in clinical needs, in line with legal and departmental requirements, and to communicate assessment and treatment results to the appropriate disciplines and patients in the form of reports and letters.
· To take responsibility for health and safety aspects of your work and implement any policies, which may be required to improve the safety of your work area including prompt recording and reporting of incidents to senior staff.
· To be responsible for equipment used in carrying out duties and to adhere to departmental policy for safety and maintenance, including competence to use equipment and to ensure its safe use by others through teaching, training and supervision of practice.
· To be responsible for the evaluation of new equipment.
· To carry out risk assessment within the service, own patient caseload, equipment and environment on a daily basis (health & safety and clinical) and to minimise risk within the team.
· To gain valid informed consent and have the ability to work within a legal framework with patients who lack capacity to consent to treatment.
Person specification
Education/Qualification
Essential criteria
- Registered Professional with NMC
- Post registration respiratory qualification or equivalent experience (combination of degree/ short courses)
- Teaching/ Mentorship qualification or equivalent experience
- Member of respiratory specialist interst group(s)
Desirable criteria
- Qualification in Independent prescribing or willing to undertake training
Experience
Essential criteria
- Experience of working with respiratory patients at a senior level
- Experience working within a community setting
- Demonstrates self-awareness and the ability to learn through reflection
- Demonstrates ability to managing change
Desirable criteria
- An eagerness to participate in service development and audit
Skills and knowledge
Essential criteria
- Ability to perform spirometry and reversibility testing and interpret the results or willing to undertake training
- Ability to perform capillary/arterial blood gas sampling and interpret the results or willing to undertake training
- Ability to be proactive and resourceful
- Ability to make decisions and prioritise actions
