Job Purpose
Working closely with the Primary Care Dietetic Team, you will provide face-to-face and virtual clinical nutrition and dietetic services in the community (including outpatient clinics and domiciliary visits to housebound & care home patients). The post holder will have their own caseload of adult patients and will focus on a range of nutritional issues including malnutrition and chronic disease.
9am – 5pm, Monday – Friday
Would consider reduced hours / part-time hours
Main duties of the job
The post holder will be responsible for:
· Provision of specialist Dietetic assessments and treatment to adult patients at health centres and a domiciliary service to patients in their own home and Care Homes within Brent.
· Management of specialist caseload of clients who are malnourished or enterally fed and to maintain associated patient records.
· Making recommendations to GPs on appropriate prescribing for their patients’ dietary requirements.
· Liaising with Dietitians and other health care professionals to ensure clients discharged from secondary care are followed up appropriately and receive ongoing input as required following discharge
· Participating in clinical governance including Quality Improvement activities within Nutrition & Dietetics Brent.

Detailed job description and main responsibilities
· To take a delegated lead on the development of service objectives or projects in liaison with colleagues.
· To assume delegated tasks as requested by Head of Nutrition & Dietetics, including participation in working groups and policy development groups.
· Organise and manage own caseload including clinical cases, health promotion and education.
· To deal with initial complaints sensitively, avoiding escalation where possible in accordance with Trust policies whilst maintaining effective communication with the Head of Nutrition & Dietetics.
· To be responsible for the security, care and maintenance of equipment (eg wheelchair scales, hoist scales) ensuring standards of infection control and safety are complied with – including equipment loaned to clients and that used for community-based programmes.
· To be responsible for ordering and maintaining department supplies of nutrition support samples, associated enteral feed equipment and ensure current information is available for their use.
· To advise Head of Nutrition & Dietetics on issues of service delivery including clinical activity, service pressures versus service capacity.
Person specification
Education
Essential criteria
- Degree or postgraduate diploma in Nutrition & Dietetics
- Health and Care Professions Council registration
- Evidence of continuous professional development
Desirable criteria
- Membership of British Dietetic Association
- Post graduate qualification in a dietetic related field (eg paediatric dietetics, health promotion, Cognitive Behaviour Therapy)
Experience
Essential criteria
- Post-registration clinical dietetic experience
- Experience of providing dietetic advice for children (age 0 – 18)
Desirable criteria
- Experience of using EMIS or a similar patient information system (eg SystmOne, RiO).

You must have appropriate UK professional registration.
