An exciting opportunity has arisen for an enthusiastic and dynamic pharmacist to join Greenwich Medicine Optimisation team. 
This is a new collaborative post and the post holder will work as part of ICB medicines optimisation team (2 days) and PCN/Practice multidisciplinary team (3 days) to enable the delivery of safe, effective informed medicines.
The NHS South East London Integrated Care Board (NHS SEL ICB) is an NHS statutory organisation. We bring together partners involved in planning and providing NHS services, working together to meet the health needs of the population within South East London and deliver the Integrated Care Partnership’s strategy. We are responsible for allocating NHS resource to deliver this plan. 
Joining an established team which has a long history of successful working relationships with our local stakeholders, you will be working closely with a range of clinicians/stakeholders to shape healthcare and NHS transformation across our diverse and vibrant borough. Through a collaborative approach with stakeholders working across the local and SEL system, the team supports safe and effective medicines optimisation functions, to enable clinical and cost-effective use of medicines for our population. 

Main duties of the job

With excellent relationship, communication, and organisation skills, you will work as part of the Greenwich Medicines Optimisation Team to deliver medicines optimisation services for the people of Greenwich. You will contribute to promoting and implementing safe, clinically cost-effective and high-quality medicines optimisation initiatives across the healthcare system. 
The post holder’s remit will be delivering and implementing medicines optimisation plans for Greenwich, working through their Assistant Director Medicines Optimisation and colleagues in other place-based teams. The post holder will work as part of a collaborative South East London medicines team which will include supporting delivery of collaborative transforming and integrating optimisation medicines optimisation areas.  
The post holder will provide pharmacy and medicines optimisation leadership in the partner organisation, helping guide the development and growth of medicines optimisation services across the PCN geography, reflecting the changes that primary care faces in the coming years, expanding the pharmacy delivery footprint of the PCN and maximising its achievements.  The postholder will support the PCN with the delivery of safe, effective systems for medicines optimisation, reducing medicines waste and maximising patient outcomes through structured medication review.
