Job overview

The Clinical/Professional Lead for Urgent Care is a key role in leading the Tameside Locality system to deliver the urgent care requirements of the NHS Long Term Plan, and to ensure safe and effect urgent care services are in place for Tameside.

 A key aim of the role is to help design and implement services that improve health and wellbeing, reduce the need for unplanned care and support relevant care pathways.

Currently this role is offered on a Contract for Services opportunity and you should be currently working with Greater Manchester GP's.

This current contractual mode is currently under review and may subject to change

Main duties of the job

·       Demonstrate a commitment to working in a collaborative manner and be a clinical advocate for driving continual improvements in urgent care; building on the established relationships with partners across GM ICS and within the Borough.

Provide clinical input to the monthly Urgent Care Delivery Board,and provide clinical leadership to the Urgent Care at Home workstream being a proactive member taking responsibility for completing assigned actions within the agreed timelines.  

·   Inform and support local implementation of national requirements, ensuring that colleagues in the locality are engaged and aware of their clinical responsibilities with regards to such requirements.

 Support initiatives which are aimed at managing urgent care demand and improving the quality of services for our population.  

·       Input to PCN developments that focus on meeting urgent and unplanned care needs  

·       Effectively work with the Tameside GP Alliance to reduce unwarranted variation in access and services offered in regards to urgent care.  

·       The role will provide challenge and clinical leadership for pathway developments, ensuring a robust interface between primary care, community, and acute services.

Detailed job description and main responsibilities

1)    To work proactively across organisational boundaries to improve health outcomes and the way services are delivered across the urgent care system.

·       To improve access to the most appropriate urgent care services within the locality ensuring that when people have an urgent care need, they can access the most appropriate services in a timely manner.

·       To reduce health inequalities and gaps in service provision

·       To consider the needs of the local population.  

2)    To provide clinical leadership for the Urgent Care at Home workstream of the urgent care delivery within the Tameside Locality by working across interfaces with:

·       Locality partners, particularly Primary Care contractors and ICFT re Digital Health, SDEC etc but also other NHS providers, Council and VCSE

·       NHS GM, including NHS GM Medical and Nursing Directorates, SCNs and NHS GM System Boards  

3)    To work in conjunction with the CCPL for Urgent Care – Acute and GM focus.

4)    To support delivery of the themes and priorities of the NHS GM Joint Forward Plan and national policies which relate to the provision of urgent care.

Education, Training, qualifications

Essential criteria
  • Be a Clinician registered with a recognised Clinical Professional Body
  • Formal Management Qualification and / or proven and significant leadership experience.

Knowledge and Experience

Essential criteria
  • Be working in a Clinical Role within the Place
  • Detailed knowledge and understanding of NHS Health and Care services at a National, Regional and local level
  • Good knowledge of the health, care and local government landscape and a good understanding of the social determinants of health and current evidence on practices which reduce health inequality
  • Extensive knowledge of the background to and aims of current healthcare policy and appreciation of the implications at a local level.
  • Knowledge and experience of good governance practices
  • Evidence of commitment to continuing professional development.
