Job overview

The Pennine Care Community Rehabilitation Team is looking to recruit a motivated and enthusiastic Registered Mental Health Nurse to join our supportive multidisciplinary team and contribute to this exciting endeavour. The successful candidate will have significant experience working with people with psychosis across a range of services.

The team supports service users to transition from inpatient rehab facilities to the community. The team has a strong emphasis on the Recovery Model, working with service users to maximise their independence and autonomy to improve their quality of life.

You will work with a team of experienced practitioners with a strong focus on psychological formulation driven, evidence based interventions.

You will receive regular supervision and your personal development priorities will be shared and supported through our appraisal process.

As this team covers the whole of the trust’s footprint, the postholder must have a driver’s licence and access to a vehicle.

We are currently based at Trust HQ in Ashton Under Lyne although this may be subject to change.  Our core hours are Monday to Friday 9am-5pm.

To work with the multidisciplinary team to offer a team approach to supporting service users either in the community or for those on the discharge pathway from inpatient settings.

To work collaboratively across a range of disciplines to engage service users in psychologically informed treatment plans.

To contribute to case formulation and treatment plans.
