An exciting opportunity has arisen for a motivated and experienced Registered Mental Health Nurse to join our specialist Early Intervention in Psychosis Team for people experiencing a first episode of psychosis as a CPA Care Coordinator.  

 

You will be joining a client centred, dynamic and friendly multi-disciplinary team, who prides themselves on delivering high quality, evidence-based interventions for those experiencing first episode psychosis.

The purpose of the role is to reduce the stigma associated with psychosis, provide an early intervention to reduce the length of time that people remain undiagnosed and untreated and to offer support and guidance throughout the persons recovery journey.

Under line management and clinical supervision from within EIT, the post holder will maintain and co-ordinate timely and appropriate interventions through mental health nursing, for a caseload of service users living in the Bolton area. The post holder will work within the framework of CPA, and in line with relevant clinical pathways as outlined within the EIT Operational Procedure. 
 
The post holder will be required to work the core hours of 9am – 5pm Monday to Friday, but flexible working will be required to meet service need. There is an expectation that the successful applicant will undertake weekend and bank holiday working on a rota basis as required to maintain the out of hours service that is offered by the Trust.

Qualifications

  • Registered professional qualification in Mental Health Nursing - RMN
  • Registration with the regulatory body NMC.
  • Willingness to participate in own continuing professional development within this specialist field

  • Post registration qualification relating to dual diagnosis.

Experience

  • Experience of working within a multidisciplinary team.
  • Experience in the application of the Care Programme Approach.
  • Effectively manage own caseload.
  • Ability to act as a care Coordinator under the CPA framework.
  • Ability to establish good working relationships with a range of providers including GP’s.
  • Ability to participate in Clinical Supervision, both as Supervisee and Supervisor
  • Maintain confidential case records.
  • Participate in multi-agency clinical meetings.
  • Involvement in Service development.

  • Ability to conduct first line assessments for individuals newly referred into the service, utilizing a biopsychosocial framework

Knowledge

  • Full understanding of the Care Programme Approach and Mental Health Act including the needs and rights of Carers.
  • Good Knowledge and application of Risk Management.
  • Good knowledge of Mental Health Act and Mental Capacity Act.
  • Proven knowledge of looking after social and health care needs.
  • Knowledge and understanding of the Clinical and Social care Governance standards.
  • Understanding of current national policy and initiatives in relation to enduring mental health issues and substance misuse.
  • Good knowledge of safeguarding procedures.

  • Knowledge and understanding of NICE Guidelines in relation to First Episode Psychosis

  • There is a requirement to travel mainly within the Bolton area and an occasional requirement to travel across the wide footprint of the Trust to attend meetings and events relevant to the role.
