Job overview

There is a new exciting opportunity for a progression post from a 5 to 6 for a care coordinator post within the integrated mental health team in HMP Styal.

The successful candidate will hold a professional qualification of OT, Social worker or RMN, under the line management and supervision from within the integrated mental health team.

The post holder will assist in the overall delivery of care in a care coordination role to patients within the mental health team at HMP Styal. You will be responsible for the assessment of care needs and the development, implementation, and evaluation of programmes of care for mental health.  This includes respecting individuality, values, culture and religious diversity of clients and colleagues and thus contributing to the provision of a service sensitive to these needs.

The successful applicant is required to demonstrate a “can-do” attitude, be resilient and possess excellent organisational skill. An ability to work well as part of a multidisciplinary team is essential. 

Healthcare services operate 7 days a week, and the successful applicants would need to be flexible in their approach to meeting the demands of the service.

The successful candidate will work towards providing the highest standard of care for the prison population ensuring consistent and effective nursing practice as well as operational activity. 

The days within the prison service are varied and there is a wide range of training and development opportunities available.

 

Main duties of the job

  • Be a full member of the multi-disciplinary team (MDT)
  • Effectively manage own caseload accepting referrals from agreed agencies in accordance with agreed operational procedures.
  • Undertake formal mental health and social care assessments of new clients within own caseload.
  • Undertake assessments of clients as part of the integrated mental health duty system in accordance with agreed operational procedures.
  • Assess, care-plan, provide interventions and evaluate and record mental health care delivered to clients on own caseload.
  • To provide effective care coordination to an identified caseload of complex service users, ensuring the delivery of evidence-based interventions to encourage successful outcomes under the Care Programme Approach (CPA).
  • Provide Specialist Mental Health Assessments, Continuing care assessments and advise management plans as requested.
  • To encourage and support clients to collaborate in their treatment programme and recovery.
  • To encourage self-help as much as possible, promoting the maintenance of skills and independence in clients.
  • To support health promotion and act in a consultative role/advisor to other community services/agencies and personnel in relation to individual clients and their mental health issues.
  • To undertake and record comprehensive risk assessments

Person specification

Experience

Essential criteria
  • Experience of working in a multidisciplinary team
  • Experience in the application of the care programme approach
  • Experience of effectively managing own caseload
  • Ability to act as a care coordinator under the CPA framework
  • Ability to establish good working relationships with a range of providers including HMPPS staff
  • Ability to participate in clinical supervision both as supervisee and supervisor
  • Maintain confidential ability to conduct first line assessments for individuals new into custody.
  • Participate in multiagency clinical meetings and Involvement in service development
Desirable criteria
  • Evidence of improving standards of care through the clinical audit cycle.
  • Experience of upholding the principles of integrated team working practices

Knowledge

Essential criteria
  • The post holder should have a full understanding of the care programme approach and the mental health act.
  • Good Knowledge and application of Risk management
  • Good knowledge of the mental Capacity act.
  • Able to demonstrate higher levels of decision-making, seeking guidance from the manager when required.
  • Ability to clinically supervise nursing colleagues.
  • Knowledge of clinical governance and application to practice.
  • Evidence of ability to plan and deliver programmes of care within a secondary care setting based on assessment findings and interpretation of available clinical information.
  • Good knowledge and application of Safeguarding procedures
Desirable criteria
  • Good understanding and application of information technology.

  • Able to demonstrate effective time management skills to plan clinical activities, training and education and staff rotas.
