Job overview
The Band 4 Maternity Diabetes Practitioner will support the delivery of high-quality care to individuals with diabetes. Working alongside the multidisciplinary diabetes team, the post-holder will provide education, monitoring, and lifestyle advice to patients, ensuring effective diabetes management. They will also contribute to the improvement of patient outcomes through support, guidance, and administrative duties. |
Main duties of the job
Work collaboratively with the Diabetes Lead Midwife and wider diabetes team to provide evidence-based care to women with diabetes in pregnancy.
Provide one to one and group education sessions for women commencing blood glucose monitoring in pregnancy or commenced on Insulin to empower them to self-manage their condition.
Support the weekly MDT clinics by undertaking insulin starts, venepuncture, urinalysis, blood glucose and ketone testing
Promote healthy living through tailored advice on diet, exercise and medication adherence
Assist in identifying patients at risk of complications and escalate concerns to senior staff when necessary.
Maintain accurate and timely patient records in line with Trust policies
Provide administrative support to the diabetes team including:
Data input, collection and collation for audits, statistics and information sharing with primary care
Emailing notifications to GP practices regarding patients on going care from the diabetes team
Assist in organising clinics including arranging appointments
Triaging telephone enquiries from patients and other health professionals
Attend and contribute to team meetings and case discussions
Work with indirect and supervision from the Lead Diabetes midwife, operating at all times within the assessed level of competence. Clear professional boundaries and limitations will be discussed and monitored by the specialist midwife

Detailed job description and main responsibilities

If successful this vacancy will require an enhanced DBS check at a cost of £54.50. This cost will be deducted from your first Trust salary
In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16.

Education and Qualifications
Essential criteria
- GCSE level English and Mathematics or equivalent
- NVQ level 3/4 or equivalent health related studies OR substantial successful experience delivering Diabetes / support
Desirable criteria
- Experience working in of Maternity care
Knowledge
Essential criteria
- A good level of understanding of evidence-based practice in relation to Diabetes in Pregnancy
- An understanding of human behaviour in relation to making healthy lifestyle changes and decisions
- Understanding of the Saving babies lives care bundle, in particular element 6
Experience
Essential criteria
- Experience of providing healthcare advice or promoting healthy lifestyle advice to patients or members of the public
- Experience of providing help, support and/or counselling on either a one-to-one basis or as part of a group setting
Desirable criteria
- Experience of providing Diabetes support
- A record of working with pregnant women
- Experience of developing and delivering successful health promotion interventions
Skills and Abilities
Essential criteria
- The ability to build and maintain effective working relationships with professionals/individuals from a variety of different professional backgrounds
- Ability to work independently and as part of a team
- Evidence of continual personal and professional development
- Excellent communication skills (both written and verbal) and listening skills.
- A good standard of computer skills
- Good organisational skills and the ability to prioritise workload and coordinate own schedule without direct supervision
- Accurate and sufficient record keeping
- A basic understanding of audit and evaluation
Desirable criteria
- Able to work as a mentor to other colleagues including teaching and providing presentations
- Experience of using databases/electronic clinical record systems
