An exciting opportunity has arisen to join Wigan living well service as an advanced nurse practitioner. 

The Advanced Practitioner will be a member of the multi-disciplinary Wigan Living Well Hub which consists of RMNs, a Pharmacist, Social Workers, Occupational Therapists, Clinical Psychology, Peer Mentors, Volunteers and Mental Health Link Workers. 

 

Main duties of the job

The Advanced Practitioner will work within a trauma informed model to engage and offer a specialist service to the service users referred to and under the care of the Wigan Living Well Team. This would include attendance at clinical multi-disciplinary team meetings, facilitating clinic appointments both out in the community and in a clinic setting, commencing and maintaining medication regimes, completing appropriate Paris documentation, and being a part of the Senior Leadership Team.

The Advanced Practitioner will be responsible for assessment, formulation and prescribing of care planning in line with the service Standard Operating Procedure and agreed protocols and pathways. The post holder will be recognised as an expert practitioner and will advise the Multi-Disciplinary team on expert, safe and effective models of care and treatment.

They will provide clinical leadership and management leading to the provision of safe, quality and effective care. They will also lead on Physical Healthcare agenda for the service and will support the improvement of workforce knowledge through education and training programme. They will support with the ongoing development of the service and developing and maintaining good relationships with wider Wigan and GMMH Manchester services

The post holder may additionally lead on practice and service developments. The post holder is required to demonstrate clinical and practice expertise via interventions, supervision and modelling good practice.

Person specification

Education / Qualifications

Essential criteria
  • Registered Mental Health Professional
  • Educated to degree level at least 2:2 at honours degree
  • GCSE or Level 2 Maths and English Language with original certificates required to enter apprenticeship programme
  • Completion of MSc in Advanced Practice and Non Medical Prescribing qualification
Desirable criteria
  • Qualifications in project management and/or quality improvement
  • Qualification in leadership / change management.

Experience

Essential criteria
  • Extensive experience in a senior clinical / practitioner / managerial role within adult acute mental health services.
  • Extensive experience of working with clients with complex needs
  • Experience of prescribing medication
  • Compiling protocols and clinical guidance, Audit and Research
Desirable criteria
  • Experience of working with disadvantaged service user groups

Knowledge

Essential criteria
  • Advanced Clinical Practice skills
  • Management of adult service users with complex needs including dual diagnosis.
  • Clinical examination skills.
  • Accountability of own role and other roles within the team.
Desirable criteria
  • Ability to undertake physical health interventions such as wound care.
  • Knowledge of issues affecting the client group
  • Knowledge of legislation affecting the client group
  • Ability to interpret physical health investigation results such as ECGs/bloods

  • Communication skills, both written and verbal
