Job overview
To work within the Learning Disability Directorate as member of the Children’s Learning Disability Team which currently sits within in the Tameside and Glossop Learning Disability Team.
The post holder will provide specialist health and behaviour assessments to children and young people with a learning disability who are referred in to the team.
Interventions will be delivered across different settings including the person’s home. There will be a requirement to liaise with and work collaboratively with partners and stakeholders to ensure responsive, effective, safe and child centred services are delivered.
Main duties of the job
• To work in accordance with Pennine Care NHS Foundation Trust policies and procedures at all times.
• To provide a safe, effective, responsive, efficient and timely community nursing services to children and young people with learning disabilities, their families and carers.
• To undertake a range of comprehensive assessments for children and young people with a learning disability and complex and challenging needs. This assessment work requires a specialist knowledge base on which to practice.
• • The post holder may be required to undertake additional training in implementing high quality Positive Behavioural Support
• The post holder will be expected to offer behaviour support training to identified others including teams around the child
• To develop a range of interventions and specialised care plans.
•
• To embrace the public health role, promoting health and wellbeing and where possible, prevent ill health.
• To support the work of the Community learning disability nurses, in reducing the health inequalities faced by people with learning disabilities.
• To work collaboratively with primary care and secondary care services to ensure the health needs of people with learning disabilities are met.
• The post holder will undertake an active role in identifying and managing risk.
• The post holder will be required to work in close partnership with Social Services and Child Safeguarding.

Experience
Essential criteria
- Registered Nurse Learning Disabilities
- Post registration training, willingness to undertake training to post graduate diploma level
- Demonstrate knowledge and experience supporting children wiht a learning disability
- Experienced in managing risk and working in partnership wiht all partners
- Knowledge of LD specialist assessment tools and of behavioural techniques
- Ability to deal with complex sensitive and distressing situations which may include challenging behaviour,abuse and breaking bad news
- Knowledge of child safe guarding
- Ability to complete complex assessments and formulations
- Experience of supporting people wiht complex and challenging needs
- Use of a car or access to a means of mobility across the trust footprint
Desirable criteria
- Trained in positive behavioral approaches
- Training in clinical supervision
- Previous community nursing experience
- Knowledge of transforming care/SEND/EHCPs
- Experience in delivering training
