Main duties of the job

- NMC Registered Nurse
- Community Specialist Practitioner degree
Qualifications
Essential criteria
- Up to date professional portfolio demonstrating evidence of reflective practice
- Ability to travel to different geographical locations across the Trust footprint in line with service needs
Desirable criteria
- Full UK driving license with willingness to use own car for work purposes
Knowledge, Skills, Training and Experience
Essential criteria
- Demonstrate excellent communication skills both written and verbal
- Ability to plan and organise own work
- Aptitude to work cohesively within a team
- Computer literacy skills Awareness of community working and care pathways
- Post Registration experience.
- V150 prescribing.
- Leg Ulcer Management qualification
- Good interpersonal skills Leadership potential
- Management of change Relevant Community experience
