Job overview

This is a fantastic opportunity for an individual with experience of working in the community, with equipment and/or adaptations, to be part of the Assessment Team in Trafford.

The post holder will be responsible for their own caseload working in peoples own homes . This will include working under the supervision of  the Occupational Therapist  to support equipment fitting, training patients and informal carers in the safe use of equipment and adaptations, liaison with other staff groups, administration tasks and undertaking joint visits with specialist companies.

You will receive training and supervision to develop your skills specific to this patient group and job role.

Main duties of the job

·       Enable patients to achieve their rehabilitation goals, by assisting them in the use and practice of skills essential to regain or maintain independence and quality of life in the community setting

·       Independently carry out visits to patient’s homes, deliver and fit equipment as recommended and complete specific rehabilitation tasks.

·       To work as an integral member of the team, participating in designated meetings and service developments

·       Be responsible for designated administrative duties, general stocktaking, maintenance and storage of therapeutic equipment.

Person specification

essential and desirable

Essential criteria
  • NVQ level II in Direct care or equivalent
  • Commitment to continuing self development with a willingness to undertake and achieve NVQ level 3 in care
  • Working in a Hospital ward or community department
  • Experience of working with patients, or members of the general public in a caring environment
  • Able to organize own workload and meet deadlines
  • Able to work with minimal supervision
  • Knowledge of essential care skills e.g. infection control, activities of daily living
  • Car owner – current driving license
Desirable criteria
  • Experience of lone working in the community setting.
  • Basic computer skills
