Job overview

To provide clinical leadership for the Home First Hub and support the wider community health and social care teams ensuring that safe and effective quality care is delivered. Secure and assure the highest standards of clinical care by the provision of clinical leadership and support for staff delivering clinical services,  service users and their relatives.

Main duties of the job

 

1.    To provide effective leadership and be responsible and accountable for the operational management, co-ordination and delivery of Home First services Home First Acute flow, Home First Hub and Home first Rehab & Reablement.

2.    You will lead a multi-professional within SCO in accordance with set objectives, local and national targets, quality standards, controls and resource constraints that ensure the delivery of high-quality patient centered services that meet                      patient needs.

3.    To be a budget holder and be responsible and accountable for managing the budget                      within given resources.

4.    To ensure all care is delivered according to Trust policies and procedures ensuring appropriate governance arrangements.

5.    To utilise clinical audit, research, supervision and teaching to monitor and improve standards of care by efficient and effective use of resources.

6.    Your multi-disciplinary team made up of AHPs,RNs, Soical Work, Administration Staff, Voluntary Sector colleagues and Housing  all involved in supporting people to return home after their discharge. The Home First Hub is centered around why not home, why not today. We want to reduce the risk of harm to the person by delays in their transfer of care and we want to improve people's outcomes, home is best.

Please note an NMC PRN PIN is not a full NMC PIN and would not fulfil the essential criteria for this role.     

This vacancy is open to those who have a current NMC PIN to practice as an Registered Nurse or Midwife in the UK, or those who are currently enrolled on a course which means they expect to receive their full NMC PIN within the next 3 months.                                                                                                                            

The NCA can no longer accept applications from International Nurses and Midwives who are yet to complete all competencies needed to receive a full NMC PIN to practice in the UK as we have paused our OSCE programme indefinitely.   • • Experience of working at a senior level in MDT team (Nursing, Social Work, AHP’s)
  • • Previous experience of implementing a Discharge to Assess or Home First services
  • • Managing a team to deliver service objectives & performance
  • • Risk management and clinical governance
  • • Experience in service transformation, process redesign and system improvement
  • • Evidence of excellent communication skills, verbal and written
  • • Evidence of Leadership Skills
  • • Good understanding of the drivers around hospital discharge and the associated legislative frameworks that support this
  • • Knowledge of SCO Priorities and objectives
  • • Knowledge of standards for better health/clinical governance and their professional implications
  • • Managing a team to deliver service objectives & performance
  • • Experience in service transformation, process redesign and system improvement
  • • Ability to work on own initiative and as part of a team
  • • Ability to affect and manage change
  • • Consistently thinks about how their work can help and support clinicians and frontline staff deliver better outcomes for patients
