30 hours per week

The post holder is responsible for the assessment of care needs, planning, implementation and evaluation of programmes of care for a defined caseload exercising a high level of clinical judgement, discretion and decision making. The post holder is responsible for the caseload management and leadership of a team of community nurses.  You will play a key role in demonstrating innovative practice, service improvement and maintaining the continued development of team members and learners in the clinical environment.

Prescribe and be responsible for the provision of nursing care by his/her nursing team within the patient’s own home, clinic or other appropriate setting during the day.

Ensure the delivery of prescribed care through effective case management and effective delegation and organisation of work to the nursing team on the current shift.

Ensure that a holistic, systematic approach to care is undertaken by the nursing team and regularly reassess the patients on the caseload.

Undertake holistic patient assessment and reassessment; develop appropriate individual care plans for patients. Implement and evaluate individual treatment plans for patients.

To practice as a Community Nurse Prescriber, maintaining competence in line with NMP policy and prescribing within own sphere of competence if appropriate.

Promote effective evidence based clinical practice, appraise standards of current practice, identify poor professional practice and take action to address it.

To provide care in a manner that at all times reflects The Code (2015) professional standards of practice and behaviour for nurses and midwives. NMC (2015)

To undertake caseload audit in relation to capacity and demand of the service.

To be responsible for the provision of nursing care by his/her nursing team within the patients own home; clinic or other appropriate setting; ensuring patients receive the highest quality of care.

To use a recognised model of care as a base for assessment.

To ensure the delivery of prescribed care through effective case management and effective delegation; to ensure consistency and continuity in the quality of care.

To ensure that comprehensive risk assessment and management strategies are incorporated into all patient assessment and care

To ensure that wherever possible engagement with patients/families/carers takes place to ensure user participation in the planning implementation and evaluation of services

To advise and encourage patients and families to develop strategies and manage their own health; to include the teaching of basic nursing care

To maintain accurate and up to date nursing records, patient confidentiality and statistical records as required by the service.

To work with other professionals to where appropriate to maintain patients within their usual place of residence.

Responsible for prioritising own work load and that of staff within the team on the shift.

The Trust is committed to safeguarding and promoting the welfare of children, young people and vulnerable adults and expects all staff and volunteers to share this commitment and promote safeguarding by implementing the Trust’s policies and procedures, acting promptly on concerns, communicating effectively and sharing information appropriately.

If successful this vacancy will require an enhanced DBS check at a cost of £41.90. This cost will be deducted from your first Trust salary

In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £13.
