Job overview
Band 6 out of hours nursing sister required for the out of hours service covering the Stockport Community. You will be visiting patients in their own homes including residential homes out of hours providing unplanned palliative care, End of life support, catheter care, symptom control and any unscheduled nursing need required by the Stockport Community to help prevent a hospital admission.
You will be a community nurse with a keen interest in Palliative care and working out of hours. You will be working within a very knowledgeable welcoming team of nurses and Health care assistants with a robust preceptorship period.
You will be liaising with out of hours Medical team Master call as required acting as an advocate for patients and their families/Carers whilst also liaising with the District nursing day teams.
You will be supporting the band 7 team lead with the day to day running of the team, alongside the overnight band 6 post holders. This role includes staff management with training, appraisals and rota.
Main duties of the job
We are looking for flexible, innovative practitioners’ with a variety of clinical skills. The District Nursing Service provides care in patients’ homes as well as residential homes caring for patients with complex needs and supporting carers.
The post holder should have good organisational and assessment skills be flexible and wiling to cover the service according to need around your role, over a 7 day period. Working with a well established team.
This post requires good communication and interpersonal skills and the ability to be adaptable, reliable and approachable. The post holder will need to be a car driver and have a car available for work with a UK valid driving licence and able to upgrade care insurance to business use.

Essential criteria
- 1st level registered nurse
Desirable criteria
- Nurse Prescriber V100
- Degree/Cert/Diploma Specialist Practitioner Nursing in the Home (District Nursing)
Knowledge
Essential criteria
- Up to date knowledge of nursing issues
- Demonstrate research based practice
- Competencies within the scope of professional practice to meet service needs
- Up to date knowledge of community nursing issues
Desirable criteria
- Ability to assess risk especially in relation to out of hours services relating to patients and staff
Experience

- Responsibility for a defined area that includes assessment, planning and implementation of care
