We are looking for an enthusiastic, innovative, experienced Physiotherapist to join our friendly, forward-thinking team providing rehabilitation for a diverse range of people with stroke and neurological conditions within Stockport.

Based in the community, you will work within a comprehensive multidisciplinary team of Occupational Therapists, Physiotherapists, Speech and Language Therapists, Clinical Psychologists, Nurses, Assistant Practitioner, Therapy Assistants and Admin. You will be welcomed as a valued member of the coordinated and committed team, sharing knowledge, skills and opportunities to learn.

You will be provide specialist knowledge to facilitate the early supported discharge of stroke patients from the stroke unit, and work with patients with acute and long term neurological conditions to provide rehabilitation in the home environment, and other community settings, improving their quality of life.

Support and education is provided with the opportunity to work directly with experienced and specialist clinicians across the team and the chance to contribute meaningfully to a fast-paced and growing team.

You will undertake comprehensive assessments and use your advanced clinical reasoning skills to develop and deliver rehabilitation programmes that support patients to achieve their goals.

You will participate actively in our weekly MDT meeting, linking with therapists from other disciplines and helping to ensure fully co-ordinated communication and goal-setting. Within the multidisciplinary team you will provide support and training to our Band 5 Physiotherapists, Assistant Practitioners and Therapy Assistants within the delivery of rehabilitation interventions and a supervisor, as well as providing clinical education to student members of the team.

As a member of the CNRS team you will have excellent assessment, communication and problem-solving skills and demonstrate an adaptable, flexible approach whilst meeting guidelines and standards.

Joining the Community Neuro Rehab Service (CNRS) in Stockport, you will see we’re a supportive, caring and proactive team. Everything we do is aimed at providing exceptional, holistic, person-centred care to people with stroke and neurological conditions within the community of Stockport.

The Trust is committed to safeguarding and promoting the welfare of children, young people and vulnerable adults and expects all staff and volunteers to share this commitment and promote safeguarding by implementing the Trust’s policies and procedures, acting promptly on concerns, communicating effectively and sharing information appropriately.

If successful this vacancy will require an enhanced DBS check at a cost of £41.90. This cost will be deducted from your first Trust salary

In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £13.
