Job overview

The post-holder will work to support the Service Design Manager with the effective commissioning of highly complex individualised care packages and will be required to undertake commissioning reviews to ensure that care and treatment objectives are clearly established by the clinical team and met within agreed timeframes. The post-holder will also work with the commissioning manager to implement the NHS Transforming Care Agenda. This is to support the delivery of appropriate care to meet identified needs in the least restrictive environment, to maintain the commissioning of person-centred and cost effective care packages as well as reviewing such as required.  They will be an active participant in the Individual Mental Health and Disability (IMHaD) Group where applications for NHS funded care are considered and where recommendations are made to the Mental Health and Learning Disabilities Resource Allocation Panel (MH & LD RAP).

Main duties of the job

The post holder will work with commissioned NHS, social care, independent and voluntary sector providers to develop wraparound packages of support that facilitate the timely and safe transition of individuals with Mental Health needs and Learning Disabilities from acute and specialist care settings.

Operate effectively in a flexible and demanding environment and proactively engage with NHS and independent sector staff, consultants, clinical teams, and contractors working on a variety of topics. Our key partners are:

Detailed job description and main responsibilities

To ensure own actions contribute to the maintenance of a quality service provision.

To be responsible for the self-development of skills and competencies through participation in training and development activities and to maintain up to date technical and professional knowledge relevant to the post.

To participate in the ICS performance and development review and to undertake any identified training and development related to the post.

To undertake statutory and mandatory training as deemed appropriate by the ICS.

To develop and maintain effective working relationships with colleagues.

To adhere to all ICS policies and procedures.

The post holder shall as necessary provide cover for and undertake duties of absent colleagues.

Safeguarding Children and Adults

All staff must ensure they adhere to the ICBs safeguarding children and adults policy and comply with the Local Safeguarding Children and Adult Board procedures. They must be mindful of their responsibility to safeguard children and adults in any activity performed on behalf of the ICB in line with the requirements of statutory guidance and legislation. Staff must keep up to date with safeguarding knowledge and skills by undertaking mandatory safeguarding training as specified in the ICB safeguarding policies, which includes understanding and recognising the signs of abuse and neglect and knowing how to raise concerns when these are identified.

Professional Registration

Essential criteria
  • Current professional registration such as NMC registration, RMN RNLD
  • Evidence of post registration education and training, supplemented by diploma level specialist training, experience, short courses
Desirable criteria
  • Leadership qualification Dual NMC registration RMN/RNLD

Qualifications

Essential criteria
  • Educated to degree or equivalent level or Nursing qualification or equivalent Occupational Health, Physiotherapist, Social Worker qualification with current registration
  • Evidence of post registration education and training, supplemented by diploma level specialist training, experience, short courses

Experience

Essential criteria
  • Experience of working in a health care environment.
  • Extensive mental health experience
  • Excellent written and verbal communication including providing and receiving complex, sensitive or contentious information
  • Negotiation and conflict management skills and the ability to influence
  • Experience completing complex case reviews and reports with recommendations

Knowledge

Essential criteria
  • Ability to review the quality and outcomes of services for people with a mental health need
  • Line management responsibility for IMHaD administrator
  • Excellent knowledge around NHS Framework for Continuing Healthcare and Section 117 aftercare Knowledge and understanding of current health and social care policy including Transforming Care Programme, and application to practice.
