Trosolwg o'r swydd

The post holder will co-ordinate and organise the inpatient and outpatient referrals for ERCP providing nursing support, information and advice to patients and their families, relatives and carers. The post holder will facilitate communication and act as liaison between the referrer, clinicians, ward staff and endoscopy unit staff.

The post holder will ensure patients and their carers fully understand the procedure for which they have been referred.

The post holder will review the referrals and assess the patients to determine suitability and priority against agreed protocols and criteria, booking the patients appropriately and in a timely manner.

To facilitate the safe efficient and optimal use of ERCP sessions. The post holder is expected to provide nurse led consent for inpatients and outpatients where appropriate.

The post holder will record activity, outcomes and prepare reports in order to evaluate the role.

The post holder will be expected to participate fully in their personal development and review process in order to achieve the skills required to deliver the role effectively and efficiently.

 To take the lead role in managing the ERCP referral waiting list for endoscopic examination following medically agreed protocols and criteria:

  • Will assess the patients suitability for the endoscopic procedure
  • Will manage the waiting list prioritising patients according to clinical needs in accordance with agreed protocols/criteria.
  • Will identify those patients who can safely be discharged and return for their procedure as an outpatient.
  • Will ensure any patient returning for an outpatient procedure leaves with a booked appointment and instructions regarding any preparation.
  • Act as point of contact for all inpatients and those discharged to return as an outpatient
  • To work with the MDT to ensure the patient is fully prepared both physically and mentally so the procedure can take place without delay.
  • Monitor/evaluate outcomes of services and care in conjunction with the departmental manager.
  • Assist with the introduction of new ideas and practices.
  • Positively contribute to the development of nursing and healthcare services within areas of responsibility.
  • In collaboration with the departmental manager, identify service developments and plan for their implementation.
  • Carry out and fulfil all delegated responsibilities
  • Promote and demonstrate innovative practice and implement changes, as required.
  • Participate in on-call rota as part of the GI Bleed service
  • To lead the ERCP lists and procedures.
  • Train new members of staff in ERCP.

  • Support ward staff in proactively managing patient, relative and carer concerns.
  • Adapt communication style according to target audience, overcoming any barriers to understanding and providing support during distressing or emotional events.
  • Ensure that patient documentation is completed correctly, appropriately and in a timely manner
  • Maintain confidentiality of staff and patients.
  • Seek support and advice on matters outside your experience and authority.

 

Responsibility for Patient Care

 

  • Provides advice to patients and their relatives.
  • Ensure nursing related documentation is appropriate, accountable and defensible as per Trust Policy.
  • Ensure that the nursing input within the ward contributes to care quality indicators.
  • Promote the use of risk assessments and health and safety management, utilising the Trust’s incident reporting system to improve the quality of patient care.
  • Assist the Ward Manager to encourage and promote an evidence-based culture in staff through the use of procedures and protocols of care within an inter-disciplinary approach.
  • Contribute to and support clinical effectiveness and innovative practice within the provision of care.
  • Participate in the implementation and delivery of the standards set within the Nursing Strategy.
  • Be involved in the consenting of patients for their procedure or intervene to get consultant consent / best interests if the patient lacks capacity.

 

Planning and organising

 

  • Ensure timely discharge planning by working and liaising with the Multi-Disciplinary Team.
  • Develop Clinical/Non Clinical Skills which will enhance patient care delivery by maximising opportunities via the Staff Development Programme.
  • Monitor and review the effectiveness of interventions with the patient.
  • With the Unit Manager ensure that the weekly staff rota maximise the safety and efficiency of available resources.

 

Responsibilities for Physical and / or Financial Resources

 

  • Support the Unit Manager to actively contribute to Trust initiatives which contribute to the efficient management of ward/department resources.
  • Ensure safe and efficient use of equipment, resources and consumables at all times including own use and correct use by others.
  • Order items relevant to practice via template ordering system.
  • Support the Business Group objectives.

 

Responsibility for Policy and Service Development and Implementation

 

  • Collaborate with clinician colleagues to develop and implement policies relevant to the ERCP care pathway.
  • Actively participate in working parties regarding policy proposals and changes as required.
  • Assist the Unit Manager in ensuring clinical/ward indicators and patient feedback is addressed and action plans developed and monitored to address any short falls identified.
  • Assist in the monitoring of standards of environmental cleanliness and challenge when not achieved or maintained.

 

Responsibilities for Human Resources and Leadership

 

  • Ensure effective communication for patients, staff, relatives, and the Multi-Disciplinary Team (MDT), both internal and external to the ward/department.
  • Support the Unit Manager in the recruitment and selection of staff, to ensure they have the competency, attitude and skills to match the requirements of the post.
  • Support the Unit Manager with the implementation of human resource policies within the service and develop a positive performance culture that celebrates successes and take steps to manage poor performance in line with Human Resources (HR) policies.
  • Assist the Unit Manager in ensuring that the environment is conducive to the education and learning of all staff and students, with support from the Clinical Practice Lead.
  • Assist the Unit Manager in ensuring that the quality of clinical placements is enhanced through supportive teaching, mentoring and coaching at all levels.
  • Ensure a positive approach to customer care through conflict/complaints handling by all members of the ward team.

 

Responsibilities for Teaching and Training

 

  • Assist in the induction of new endoscopy staff.
  • With the support of the education and training department, assist in the delivery of the training at ward level and include assessment for competency ensuring staff are safe in care delivery.
  • Ensure training files are maintained.
  • Support the development of clinical supervision and preceptorship of nursing staff within the ward/department.
  • Develop and update patient information resources.
  • Participate in training and development relevant to the role.
  • Act as a resource for Junior Doctors

 

Responsibilities for data and information resources

 

  • Assist in the development and maintenance of staff rotas
  • Assist the Unit Manager in monitoring of the performance of the ward/department and produce reports as required.
  • Ensure the effective use of I.T. initiatives within the ward/department.
  • Create and produce reports on a regular basis which indicate the number of patients awaiting in-patient endoscopy procedures and the activity undertaken each month.

 

Research, Development and Audit

 

  • Actively participate in regular clinical audit.
  • Work with the Unit Manager to maintain and evaluate outcomes of service/care provided from their area and take appropriate action.
  • Implement research based ideas to ensure care development.
  • Support the implementation of new and innovative methods of practice within speciality and scope of practice.

 

Physical Skills and Effort

 

  • Standard keyboard skills
  • Understand and utilise all hospital booking systems such as PAS, Solus and Evolve to schedule patients
  • To safely use clinical equipment in order to carry out the job role.
  • To administer intravenous injections, syringe pumps and infusions
  • Use of physical effort to carry out the demands of the job role (e.g. pushing and pulling trolleys/ support to manoeuvre patients).
  • To undertake the necessary training to ensure safe use of medical devices and equipment

  • If successful this vacancy will require an enhanced DBS check at a cost of £41.50. This cost will be deducted from your first Trust salary

In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £13.
