Inherited Cardiac Conditions Lead Nurse Specialist

As the largest single site specialist heart and chest hospital in the UK, we, at Liverpool Heart and Chest Hospital, have a clear vision 'to be the best cardiothoracic integrated healthcare organisation'.

We provide specialist services in cardiothoracic surgery, cardiology, respiratory medicine including adult cystic fibrosis and diagnostic imaging, both in the hospital and out in the community.

We serve a catchment area of 2.8 million people, spanning Merseyside, Cheshire, North Wales and the Isle of Man, and increasingly we receive referrals from outside these areas for highly specialised services such as aortics.

We have an exciting opportunity to join an established regional Inherited Cardiac Conditions service, in a new Inherited Cardiac Conditions Lead Nurse Specialist role. The post will be based in Liverpool Heart and Chest Hospital and in Community Hubs.

An exciting opportunity has arisen for an enthusiastic, motivated  and experienced senior Nurse  to join the ICC service  at Liverpool Heart and Chest Hospital.

As a band 8A ICC lead Nurse Specialist you will proactively manage your own caseload of patients with pathology and those attending for screening, and work with partners across professional, operational and system boundaries to improve the provision  of the ICC service.

The clinics will be held onsite, in community hubs and over the telephone.  The post holder will be involved with the running and delivery of the drug provocation service. 

The post holder will always act within the NMC’s Code of Conduct, working within clinical and professional protocols.

The post holder will provide a high-quality service within Liverpool Heart and Chest Hospital and Community Hubs to all individuals and families dealing with a diagnosis or undergoing testing related to Inherited Cardiac Conditions and will be responsible for co-ordinating their pathway from time of referral onwards.

In addition, the post holder will be expected to participate in the strategic development of the Inherited Cardiac Conditions service.

·         Being recognised as an expert in their field, holding additional specialist knowledge and experience

·         Providing leadership in clinical practice for the professional team and peers and ensuring effective learning experience for students through provision of an educational environment.

·         Actively disseminating best practice through presentations and participating in nursing research and national working parties.

PRINCIPAL ACCOUNTABILITIES

 

·         Investigation for a possible cardiac genetic disorder

·         Counselling so that patient can comprehend quite often complex medical facts, including the diagnosis, probable course of the disorder and the available management.

·         Appreciate the way in which inheritance contributes to the disorder, and the risk of recurrence in specified relatives.

·         Understand the alternatives for dealing with the risk of recurrence.

·         Guide and provide information for patients to assist them in the decision-making process during treatment/action which seems to them most appropriate.

·         Support them through investigations/treatment where indicated.

·         To act as an educational resource in the field of cardiac genetics

·         Utilising advanced clinical skills in the assessment of patients including taking a clinical history, a detailed family history, perform clinical examination, order and interpret diagnostic tests, prescribe and initiate medication and use advanced clinical reasoning to produce an appropriate treatment plan.

  •       Maintain multi-professional relationships to enable the seamless flow of the service and patient pathway.
  •       Maintain databases and research documentation as required.
  •          Undertake administrative tasks as required including patient follow -up.
  •        Provide expert advice and clinical practice in relation to the specialty.
  •       Maintain appropriate channels and styles of communication to meet the needs of patients, relatives, carers, managers, peers and other professional agencies.
