Job overview
An exciting opportunity has arisen for a highly motivated cardiovascular disease nurse to join our Strategic Partnerships team, working with the Cardiovascular Disease Prevention (CVD-P) Service on a 12 month fixed term contract or a secondment.
The successful candidate will be expected to provide comprehensive assessment for patients identified with increased risk of developing cardiovascular disease. They will be responsible for the management and care of patients at risk of CVD including making observations of the patient’s physical presentation, such as blood pressure, heart rate, body mass index, point of care testing, psychological and emotional state during any reviews and report back to the clinical team. They will ensure that a high standard of patient care is provided, and that the principles of evidence based practice and best value are integral to practice and the core business objectives, in improving services for users in line with the patients vision.
You will need a positive outlook, with a desire to deliver quality service in a fast-paced environment. We are looking for someone who has a flexible approach with the ability to work on their own initiative and as part of a team, excellent organisational and time management skills and the ability to work under pressure and to tight deadlines.
Main duties of the job
The post holder will be expected to provide clinical knowledge, demonstrating a sound understanding of the issues related to the identification, assessment, treatment, and management of patients at risk of developing cardiovascular disease (CVD). The post holder will travel across Cheshire and Merseyside to deliver the service to patients in their local area. The postholder will support the development, and lead on the day to day running of the newly formed CVD prevention service as part of the strategic partnerships team. They will manage the caseload referred to the service, monitor and improve standards of care through, clinical audit, evidence-based practice, supporting professional colleagues and the provision of skilled professional leadership. They will actively contribute to the clinical governance processes within the service.

Detailed job description and main responsibilities
Clinical practice
Responsibility for assessment, planning, and developing an individualised management plan of care for patients within the CVD Prevention service
· Clinically assess patients referred with incidental findings or multiple risk factors of CVD in a community clinic setting.
· Refer patients for further investigation, with the support of the MDT and manage the patient though the service.
- Assist in developing assessment tools, and pathways in accordance with NICE guidance.
· Act as a source of expertise and point of contact for the team, liaising with health care professionals, and other service that are caring for the patient, using developed communication skills.
· Educate patients and their families about their condition.
· Liaise with the multi-disciplinary team to discuss patient care pathways
· Liaise with the adult Familial Hypercholesterolemia / Diabetes and other specialist teams across the ICB region to provide a pathway for raising awareness
· Work with primary care staff, regional consultants, physicians, nurse specialists and other related professionals to develop high levels of care for patients at risk of CVD
· Actively engage with patients to obtain feedback from patient and family experience
· Work as part of multidisciplinary team to identify and refer patients identified with a diagnosis of FH or diabetes to genetic counsellor if required
· Undertake opportunistic diagnostic testing for the public and certain sectors using point of care testing. This will involve discussing information to inform and counsel individuals on the significance of such investigations both for the individual and their family members and relay information to the relevant healthcare professions
· Provide advice and written information as required in response to clinic appointments.
Education & Development
· Keep up to date with information on national and local level issues which are pertinent to CVD and its prevention and to disseminate the information and provide education to practice nurses, other primary care staff and other care staff as appropriate
· Act as a resource building awareness, expertise, and knowledge
· Provide professional support and participate in organised study days to provide clinical expertise and appropriate education and training to all those professionals involved in providing care, across primary, secondary, and tertiary care
· Update own personal knowledge and skills by accessing appropriate and relevant seminars, study sessions, conferences, and specialist network groups
· Develop and evaluate teaching/learning resources
· Ensuring that the child, young person, and family are given the information necessary for making informed consent about treatment.
Have the ability to problem solve in the clinical environment, including the management of patients requiring telephone advice and liaise with the wider team or surgical team when appropriate.
Research & Development
· Liaise with and provide appropriate data for the service
· Develop evidence-based protocols, clinical guidelines, and policies to ensure consistency in management.
· Work collaboratively with other members of the multidisciplinary team on relevant research developments
· Support and contribute to the collection of data for related local, Trust wide and national quality indicators

Qualifications & Education
Essential criteria
- 1st level registration with NMC
- Clinical / physical examination or clinical history taking qualification (or willing to complete the course in the first 2 months in post)
- At least 2 experience within CVD speciality
- Degree level education or working towards or equivalent
- Evidence of commitment to continued professional development
Desirable criteria
- Teaching and assessing qualification from NMC or equivalent
Knowledge & Experience
Essential criteria
- Knowledge of development and treatments for cardiovascular risk factors
- Evidence of research and audit skills
- Experience of providing advice and training to GP’s and other healthcare professionals
