Frailty Advanced Clinical Practitioner

Band 8a

Job overview

As part of the innovative and ongoing development we are looking for an experienced practitioner to join our Acute Frailty Team.

As the population ages increasing numbers of older people living with frailty are presenting to ED departments. 5-10% of all those attending EDs and 30% of patients in AMU are older and frailer. People living with moderate to severe frailty suffer most hospital acquired health care problems with around 20% of patients accounting for 80% of events and disproportionate bed day use. It is recognised that by targeting this high risk group as early as possible during admission provides an opportunity to improve quality of care and optimise patient and service outcomes for a vulnerable population.

The role  involves working in the Emergency Department, Acute Medical Unit, and the Acute Integrated Frailty Unit (AIFU), as well as the development of rapid access and ambulatory clinics

Excellent and advanced clinical knowledge of caring for those identified as frail is essential with the determination and drive to make a difference.
