 Advanced Clinical Practitioner
An exciting opportunity has arisen for an experienced nurse to join the Gastroenterology Care Group as an Advanced Clinical Practitioner in Clinical Nutrition. The successful candidate will provide a specialist service to advise and promote evidence-based practice in the care of all patients receiving nutrition support.
Working autonomously and alongside the clinical lead for nutrition they will be part of the complex nutritional needs MDT, joining an experienced and highly motivated team including nutrition nurses, a dedicated nutrition pharmacist and dietitian. The successful candidate will be responsible for supporting the nursing assessments of patients care needs and support with the development, implementation, and evaluation of nutritional programmes of care.

This is a new post you will:
Manage the Nutrition Nurse Service and contribute to service development.
Attend the Feeding Issue MDT which expects referrals from around the region.
Support patients who are receiving enteral and parenteral nutrition in hospital.
Provide regular outpatient clinic for nutrition support across site (all three hospitals).
Develop a pathway to support patients who attend the Emergency Department with feeding issues to help provide care/signposting and prevent unnecessary admissions.
Deliver education and training for patients and staff in regards to nutrition.
Work with the transition team to ensure safe and smooth transition of paediatric patients into the adult services.
Work with Clatterbridge Cancer Centre to support and enhance the skills, training of their Nutrition Nurses and support patients at CCC in need of enhanced nutritional services 
Work with other members of the Nutrition Team to ensure the Intestinal Failure National register is maintained and updated on a regular basis.
Ensure all Nutritional device related policies and patient information leaflets are up to date and evidence based by the Nutrition Team

Detailed job description and main responsibilities

 
They will provide specialist nursing support and management of patients receiving total parenteral nutrition, and assessment for and management of those receiving enteral tube feeding. This will include IV access insertion and management, endoscopic enteral tube placement, clinical decision making regarding parenteral and enteral nutrition support.
The post holder will motivate staff to provide high standards of care by acting as a role model. They will provide expert professional and clinical care advice to the patients, carers, and multi-disciplinary team. They will be responsible for providing specialist education and training programmes for health care professionals and other professional groups.
They will be responsible for ensuring a robust system is in place for data collection and audit.
Their expertise and skill will enable the role and service to develop as a result of annual audit, reflection of practice, outcome from datix incidences and innovations and advances without their practice.

Qualifications

Essential criteria
  • Clinical Professional Qualification. Degree level with appropriate registration
  • MSc Advanced Clinical Practice (ACP) or started the MSc ACP pathway/programme and committed to completing within an agreed time frame: Note: Post holder will be in a training post (Annex 21) until evidence of completion of MSc ACP is provided.
  • Teaching and assessing qualification
  • Relevant post registration qualification/equivalent
  • Prescriber
  • Evidence of participation in audit and research
