To provide comprehensive, efficient and effective administrative service within a Transfer of Care Hub to support the discharge system and data outputs as required by its multi-Disciplinary teams both internal and external.

Using your organisational, analytical, administration and people skills, you will contribute towards an effective and consistent level of service.
Ensure the delivery of the quality strategy within the designated area incorporating establishment of systems and processes.

The role provides a fantastic opportunity to develop within an ever expanding service working with differing health care professionals and senior team members to help shape service improvements.

Swydd ddisgrifiad a phrif gyfrifoldebau manwl

1. To be able to update, collate and share complex discharge data within the service unsupervised and to be able to use your own initiative so other services can operate with the data you provide.
2. Organise and prioritize daily tasks unsupervised using my own initiative to ensure all duties are completed.
3. To input and maintain information on the service’s various systems, ensuring that it is accurate and up to date.
4. Creating and maintaining data logs using MS Excel and linked NHS systems.
5. Responsibility of creating new records and monitoring patients who have been added to or removed from discharge logs.
6. Receiving data, reinstating previous record, or adding a new referral for current record ensuring all information is correct and accurate, ensuring we keep within KPI time frames.
7. Responsibility for data entry including retrieval and maintenance of patient records.
8. Dealing with a high volume of queries via secure email net account. liaising with both internal and external agencies, to respond in an effective time frame using your own initiative and knowledge.
9. Effective liaison with multi-departments staff, patients, carers, and outside agencies ensuring accurate information is communicated as and when required, including reception, switchboard duties dealing with enquiries from healthcare professionals.
10. Answer the telephone, clear answerphone, record messages and respond to queries in a timely manner ensuring effective communication in line with local policies for nonclinical information only.
11. Collecting and inputting data on to the Emis Web system, investigating and acting on discrepancies to maintain accuracy.
12. To participate in service development activities and mandatory training as required
13. Assist with the training of new clerks within the service until new staff know they are competent with in all tasks required, including Emis training and data spreadsheet supporting new colleagues with advice on procedures and dealing with sensitive information.
14. Liaise with colleagues at all levels both within the teams across other agencies and organisation.
15. Manage our own workload and prioritise tasks unsupervised to ensure all duties are completed daily.
16. Operate and maintain information systems with specific responsibility to ensure high standard of information recorded is maintained in line with Trust policies and Procedures.

18. Produce standardised computer-generated reports and/or statistical information as requested by line manager. Always ensure the strictest confidentiality regarding both patients and trust, in accordance with the data protection act.
19. Transfer relevant information to all health and social care providers as requested by line manager.
20. Input accurate and timely information required for audit purposes.
21. The post holder shall as necessary provide cover for and undertake duties of absent colleagues.
22. Support individuals’ equality, diversity, and rights.
23. Contribute towards service improvement initiatives to enhance quality of patient care.
24. Provide information and support to service users/carers as appropriate.
25. Contribute to the continued improvement and quality of the administrative support service.
26. Recognise and respond appropriately to urgent situations.
27. Contribute to the effective and efficient use of resources.
28. Understand and adhere to trust policies, procedures, and guidelines.
29. Report any concern regarding patient care to line manager.
30. To undertake any other delegated duties as appropriate and commensurate with the post.

Qualifications
