Job overview

The Urgent Treatment Centres (UTC) across Liverpool are recruiting for an  Advanced Clinical Practitioner. This is an exciting opportunity for the successful candidate to  practice at an advanced level in a fast-paced environment .

The  UTC service delivers care across Liverpool sites . The service is open from 8.00 – 20.00, 7 days per week and offers flexibility with shifts patterns.

You will be the Advanced Clinical Practitioner on duty to advice and support clinical decision making and function as the point of escalation as shift coordinator. You will have responsibility for taking charge of the shift and supporting management of service provision in the absence of the Services Leads.

The post holder must show commitment in supporting and in the development of others to ensure a consistent, quality driven delivery of service, mentorship, and clinical supervision.

Main duties of the job

You will be expected to work autonomously, be highly motivated and deliver an advanced level of clinical practice within the recognised remit, working within the local and nationally agreed frameworks for Advanced Clinical Practice (ACP). This post requires the post holder to have excellent communication skills, interpersonal skills and able to manage challenging and difficult situations.

Within the role, you will be applying your advanced skills in history taking, extended assessment, clinical examination, clinical diagnosis, treatment as appropriate and referring on to other services if required. It will be expected that you will support and mentor other clinical staff to develop competencies for development.

Clinical leadership is integral to the role, you will be the clinical lead practitioner on duty to advice and support clinical decision making and function as the point of escalation as shift coordinator. You will be responsible for ensuring patient flow throughout the department is smooth and ensures good patient experience.

Detailed job description and main responsibilities

To be professionally and legally accountable for all work undertaken to practice at an advanced level of professional autonomy and accountability that is within Trust policies and the Nursing and Midwifery Code of Conduct (2008) or the Health & Care Professions Council (HCPC). 

Deliver and participate in the clinical care of patients accessing the walk-in centre. 

Prioritise workload and clinical interventions as appropriate. 

Provide a level of advanced and comprehensive health assessment based on a specialist body of.

To appropriately assess, examine, investigate, diagnose, and treat patients, resulting in the safe management and appropriate referral or discharge of patients. 

Ensure patients are informed and consent is gained prior to treatment, investigation, and management as appropriate. 

Take responsibility for the supervision/development of junior members of the team. 

Is able to identify the need for appropriate diagnostic tests, interpret the results and act on any results requiring clinical intervention or management. 

To identify and intervene proactively where circumstances contribute to an unsafe environment for patients, staff or relatives and escalate these to the health and safety team/safeguarding team. 

To share knowledge and expertise, acting as an expert resource to others across primary and secondary care as appropriate.

Contribute to the development and improvement of systems and processes that facilitate patient flow, facilitating timely discharge and preventing hospital admission. 

To improve the quality of the patient experience by identifying and meeting the individual clinical needs of patients including issues pertaining to child protection & vulnerable adults. 

Must have completed the Independent Non-Medical Prescribing course and registration with the NMC or HCPC, undertake non-medical prescribing in accordance with the Trust non-medical  prescribing policy and maintain evidence and competence of this qualification. Adhere to CPD requirements of the Trust in relation to maintaining evidence of on-going competence.

To maintain up to date patient records in accordance with Trust policy and professional bodies standards. 

Ability to adapt specialist clinical knowledge and skills to different clinical settings and influence service delivery and patient care. 

Promote and adhere to the principles and practice of the Trusts clinical governance framework. 

Contribute to the review and development of clinical patient pathways and the development of the service, ensuring our services are safe. 

Adhere to trust infection and prevention control policies and procedures. 

Maintain up to date qualifications in relation to Intermediate life support (PILS). 

Lead on the review and implementation of appropriate NICE guidance. 

Will support junior staff with clinical/therapy competencies. 

Will be the key link for other teams within the service line. 

Will be key link person for educating staff around a specific area of expertise. 

Lead on clinical audits. 

Act as a link for secondary care to discuss potential patient admissions through collaborative working.
