The Community Neuro Rehab Team is a multidisciplinary team working to provide specialist Neuro Physiotherapy, Occupational Therapy, Speech and Language Therapy, and Vestibular intervention to adults (18+) in appropriate community settings. We maximise independence and aim to provide patient centred, evidence based holistic care.
An exciting opportunity has arisen for an assistant practitioner to join the team. If you’re motivated, with therapy experience and skills, a passion to be part this service, and share our aim for perfect care, we’d like to hear from you.
The role involves working across Southport and Formby to provide support to our patients with neurological conditions in their homes and other community settings, which will enable you to develop skills and flourish within your career. If this interests you, come and join our fantastic team.
We’re a very supportive team with physiotherapists, occupational therapists and assistant practitioners.

Work collaboratively within the team and with other agencies to deliver local and national health priorities and objectives.
Be accountable for own clinical practice.
Deliver care in line with national quality standards and Trust policy.
Work within the skills and competencies expected of the role.
Act as an advocate and champion for clients.
Support clinical staff in managing long term conditions.
Lead on health promotion activities as required.
Undertake clinical tasks, following patient specific direction.
Communicate effectively with patients and carers recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.
Participate in individual clinical supervision.
Participate in audit as required by the organisation.
Undertake all relevant mandatory training.
Travel across the Sefton region to deliver care in patient's homes & community settings.

Qualifications

- Foundation Degree or equivalent or equivalent work based experience.
Experience

- Assessing, planning and implementing individualised programmes of care.
- Understanding local and national influences on health care delivery, including Public Health.
- Experience of previous work in health care.
- Clinical Governance and Risk Management.
- Actively Involved in professional development.
Knowledge / Skills / Abilities

- Knowledge of Health Promotion strategies.
- Excellent communication, organisational and interpersonal skills.
- Ability to travel, for work purposes.
