Occupational Therapist - Discharge to Assess

Are you a confident Occupational Therapist who would like to explore a more rewarding clinical challenge within a Discharge to Assess Therapy Led service?

We have a fantastic opportunity for Band 6 Occupational Therapists  to join our pioneering team as part of the wider acute therapy team at Southport Hospital.

The Chase Heys New Directions, Discharge to Assess Therapy Team, offers a dedicated seven-day enhanced therapy service to 14 beds located within Chase Heys providing specialist assessment and intervention including discharge planning to adults following their acute stay in hospital. 

Our team work closely with Trust colleagues, patients and families to achieve the best possible level of independence and quality of life, always supporting a discharge home where possible. Thus promoting the HomeFirst pathway and encouraging people's functional independence to stay safe in their own environment. 

The successful applicant will be part of a motivated, forward thinking, multidisciplinary team of Occupational Therapists, Physiotherapists, Therapy Assistants and Assistant Practitioners. The team work closely with the wider MDT to include the Care Team at Chase Heys, Medical and Social Care colleague to provide excellent care and outcomes. 

Continuous evidence-based practice and service development is fundamental to the team, and you will be passionate in identifying, leading and driving forward new projects.

You will be joining a recently established team and will have access to support and supervision from a Band 7 Team Lead, with wider clinical supervision provided through peer support from Trust colleagues. We have an excellent staff supervision system enabling our staff to develop both the service and themselves. The appropriate candidates will also demonstrate a strong commitment to training rotational therapists, rehabilitation assistants and undergraduate staff. 

 The Successful Candidate should:

  • be a registered Occupational Therapist with the HCPC (Health Care Professions Council)
  • have a minimum of 2-3 years post-qualification experience
  • have an in-depth knowledge and understanding of Occupational Therapy process / interventions and an ability to apply these skills
  • have experience of and eagerness for continued professional development
  • have experience of service development, audit and research
  • work effectively as part of a team
  • have excellent communication and time management skills
  • be passionate about student education

The Secretary of State has approved the partnership between St Helens & Knowsley Teaching Hospitals and Southport & Ormskirk Hospitals.

KEY DUTIES

  • To be professional and legally responsible and accountable for all aspects of your own work including the management of patients/clients in your care.
  • To ensure a high standard of clinical care for the patients/clients under your care and support more junior staff to do likewise.
  • To undertake a comprehensive assessment of patients in a timely manner
  • To take delegated responsibility from the Team Lead as required.

  • To promote enhanced moving and handling skills including moving with dignity
  • To have the ability to undertake self-directed/lone working in a variety of locations.

CLINICAL & PROFESSIONAL RESPONSIBILITIES

  • To maintain Professional Registration
  • To work within Trust clinical guidelines, HCPC and Registrant Body guidelines and to have a working knowledge of national and local standards and monitor own and others quality of practise against these.
  • To be responsible for a designated area of work, as delegated by a senior clinician. Plan and organise efficiently and effectively patient/client caseload, and use of time for self and more junior staff.
  • To be responsible for equipment used in carrying out clinical duties, and to adhere to policy, including competence to use equipment and to ensure the safe use of equipment by others through teaching, training, and supervision of practice.
  • To provide spontaneous and planned advice, teaching and instruction to relatives, carers, and other professionals to promote understanding of the aims of the pathway and to ensure a consistent approach to patient care
  • To train and supervise more junior staff and students. This will include the use of formal appraisal documentation.
  • To communicate effectively with patients and carers to maximise potential and to ensure understanding of their condition.

  • HCPC registered
  • Fieldwork/practice educator qualification

Knowledge and experience

  • Demonstrates substantial experience since qualifying; including experience within the acute and rehabilitation services
  • Experience of being flexible and adaptable to manage the demands of working within the current challenges of healthcare
  • Experience of mentoring, supervising, teaching and training
