Job overview

An exciting opportunity has arisen within the ICRAS Social work team (community division)  for a community assessor role to help support the service. It will involve working across the city, to undertake assessments including initial assessments of care needs for adults in their own homes, with acute, chronic and mental health conditions interpreting eligibility criteria. Arrange services for adults and/or their carers. To monitor and review as required.
Utilising persuasive and empathetic skills for service users and or their families to accept support services.
To use these skills with other professionals to enable a greater understanding and collaboration, dealing with areas of conflict and differing opinion.
Review ongoing care needs explaining to patients that services may be reduced or
withdrawn using empathetic and persuasive negotiating skills.
To look at a variety of options to meet care needs taking into account individual needs, least invasive and having to consider cost implications for both the people concerned and the public purse.
To work as part of a multi-disciplinary team ensuring that assessment and care
management is service user focused and crosses health and social care boundaries.
To contribute to the development of services by offering views and ideas to working groups to ensure the continuing provision of a high standard of care to service users.

Person specification

Qualifications

Essential criteria
  • NVQ Level 2 Social Care or Equivalent
  • A willingness to learn and a commitment to participate in training including participation in NVQ level 3 programmes

Skills

Essential criteria
  • Ability to complete an assessment of the social care needs of older people
  • Ability to arrange services to meet identified needs and to monitor, review and respond to changing circumstances
  • Ability to keep appropriate records and to provide relevant reports
  • Ability to plan and organise own work and work as a member of a team
  • Ability to work as part of a multidisciplinary team and other agencies

Knowledge

Essential criteria
  • Working knowledge of relevant legislation including, The Care Act and Mental Capacity Act
  • Knowledge of the role and Social Services/Health Services, in assessment and the provision of services
  • Knowledge of the Safeguarding Vulnerable Adults procedures
  • Experience of working in partnership with individuals and organisations engaged in Social Care.
  • Experience of working with adults with disabilities
