You should possess excellent organisational, communication and interpersonal skills together with an ability to work effectively both independently and as part of a team. An in depth knowledge of the current health agenda relating to children’s services is essential.

You will be joining a proactive skill mixed team working together to deliver the Healthy Child Programme . As part of the 0-19 Service which with other local statutory, health and voluntary agencies to provide includes Health Visiting and School nursing you will be working closely accessible, person centred packages of care in response to individual and local need. You will be working in partnership with Wirral Local Authority to take the local children’s agenda forward.

Works across Wirral supporting delivery of the Healthy Child Programme 0-19.
Manages a delegated active caseload, working as a member of a skill mixed team, responsible for interacting constructively with team members and organising, prioritising and planning their work. This includes, arranging visits and supporting delivery of universal services and additional packages of care. Facilitation and delivery of clinics and group sessions as required, including evenings based on service need.
Undertakes Healthy Child reviews/contacts and promotes uptake of immunisation as per Trust and professional guidelines.

Supporting delivery of identified packages of care, providing support and advice to clients ensuring family Health Visitors/School Nurse is informed of progress and outcomes.

Provides a holistic approach to promoting health and wellbeing whilst undertaking unsupervised contact, recognising when to liaise and report to Health Visitors/School Nurses and Team Leader
