Job overview

An exciting opportunity has arisen within the Halton Community Learning Disability Team's Intensive Support Function (ISF) for a Support Worker to support the delivery of specialist health support which promotes the independence, community inclusion and positive reputation of people with learning disabilities. 

The post holder will assist in the delivery of care plans and programmes by - implementing them and reporting outcomes, contributing to the recording of plans/programmes and providing therapeutic activity where agreed and appropriate.

Acting to as a nominated worker for individual clients who have learning disabilities and may also have extreme challenging behaviour including physical and verbal aggression, changes in mental health, forensic needs, complex physical health needs. Facilitate and support the development of appropriate personal care and independence with activities of daily living. Support self-help, communication and social skills acquisition

Main duties of the job

The service model is one of supporting people, whose behaviours challenge services, working closely with specialist practitioners and other health professionals to support this complex group of individuals to remain in their community setting and, where an appropriate admission has occurred, to be discharged promptly and safely in line with national Transforming Care strategy.

Working alongside Clinical Psychologists, Occupational Therapists, Speech & Language Therapists, Physiotherapists, Consultant Psychiatrist, Nurses and Assistants of each discipline. 

We are looking for a confident individual who will be able to provide this support and in return the post holder will receive supervision, appropriate training and CPD opportunities and an annual personal development review.

Detailed job description and main responsibilities

JOB PURPOSE

The post holder will work under supervision as part of the multi-disciplinary team and will assist and support the clinical team(s). This may include working within the patient's home environment and other clinical settings in the community.

Ensure the delivery of the quality strategy within the designated area incorporating establishment of systems and processes.

PRINCIPAL RESPONSBILITIES

1. Work as a member of the multi-disciplinary team.

2. Undertake a range of patient interventions within the scope of NVQ level 3 skills for patients as delegated by service lead or other qualified clinical staff.

3. Offer advice to patients and carers on a wide range of health issues to include health promotion, lifestyle advice and self-care.

4. Plan and organise own work schedule once delegated from service lead.

5. Undertake basic patient observations.

6. Work within existing clinical guidelines, policies and infection control procedures.

7. Offer advice and support to patients and families in a wide range of situations.

8. Communicate factual and sensitive information within confidentiality guidelines to patients, ensuring all information is appropriately documented and reported back to the service lead.

9. Demonstrate own role to new starters and students.

10. Communicate information to multi-disciplinary team and other agencies as appropriate.

11. Undertake further training to gain skills in other areas.

12. Support patients with the prescribed Pulmonary Rehabilitation exercises under the direct supervision of the Physiotherapist.
