Job overview
We are excited to invite Advanced Practitioners with suitable relevant experience to join our Community Resource Team in Bridgend as the Senior Advanced Practitioner of the Acute Clinical Team.
The Acute Clinical Team is a rapid response service that aims to avoid admissions to hospital and respond to people experiencing a functional/frailty crisis at home. The service manages adults and the majority of service users are older adults living with frailty syndromes and multiple health problems.
The team has an established group of expert multi-professionals supported by a dedicated resource of Consultant Physicians. Our integration into CTMU Health Board provides exciting opportunities and challenges to establish the model of the service across the wider Health Board area.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply
Main duties of the job
The post holder will lead the Acute Clinical Team, the rapid response, multi-disciplinary element of the CRT. They will both clinically and managerially lead the service and support the team of Advanced Practitioners to manage a caseload of acutely unwell and/or acutely functionally impaired patients.
The Senior Advanced Practitioner will act as the initial senior clinical point of escalation, supporting and advising the team with clinical problems and when appropriate, respond to patients in their home to undertake clinical assessments and reviews. The Senior Advanced Practitioner will maintain a close relationship with the Consultant Physicians to safely manage the caseload.
The Senior Advanced Practitioner will also act as the overall manager for the ACT and CRT Nursing, providing line management to senior staff, supporting operational leaders in staff management and working with the wider team of senior staff and managers throughout the CRT to co-ordinate the overall provision of the CRT services.
The Senior Advanced Practitioner will also need to engage in service design projects, workshops, network with multiple key stakeholders throughout the health service, private and education sectors to further practice development and push the agenda for growing and developing this intermediate care service. Growing awareness of the service to ensure good service uptake and evidencing the effectiveness of the service to inform the future business plan and growth of the service.

Qualifications and Knowledge
Essential criteria
- Professional Registration (NMC, HCPC)
- Level 7 level Education (Masters) relevent to area of practice
- Advanced Specialist Knowledge of patient pathways
- Advanced theoretical knowledge and practical experience to carry out clinical consultation and/or examination relevant to area of practice.
- Independent Prescribing qualification
- Evidence of maintaining an Advanced Practice Portfolio
Desirable criteria
- Management/Leadership qualification or possess equivalent skills and experience
Experiance
Essential criteria
- Extensive post registration clinical experience in advanced practice within speciality area.
- Evidence of performing at Advanced Level of Practice.
- Proven track record of working collaboratively with multi-disciplinary teams.
Skills and Attributes

- Clinical skills competence for environment/ setting.
- Demonstrate leadership/ supervision experience.
