Job overview

To work as part of a team of integrated professionals focussing on the needs of adults living with dementia and attending day services or supporting inclusion community opportunities.

To work with care providers and family members, supporting adults living with dementia, in the development of care plans that support appropriate care. 

To enhance carer resilience and education. To ensure adults living with dementia live their lives in the way they wish too ensuring voice and choice for individual. To aim to avoid unnecessary hospital or care home admission.

Main duties of the job

To upskill the existing day service provision for people with dementia, so that they become specialised to deliver services for those people with complex needs as a result of living with dementia or cognitive impairment who would not be able to access universal resources within their communities.

•To provide support for people and their carers by developing pre-emptive health and social care assessment and interventions to maximise people’s health and well-being in a supportive setting, as well as anticipatory/contingency planning approaches with carers to support them in their caring roles

•Those living with advanced dementia and their carers and families are able to receive person-centred coordinated care and support which is flexible and responsive to individual need 

The ability to speak Welsh is desirable for this role; Welsh and/or English speakers are equally welcome to apply.
