Job overview

This is an exciting time to join our integrated team of Social Workers, District Nurses, Occupational Therapists, Physiotherapists, Community Mental Health Nurses , speech and language therapists, dieticians, experienced technicians and dementia support workers. .  We are seeking to appoint an energetic, enthusiastic full time  permanent  community mental health nurse  to this team. .

 The team works with adults from the age of 18, into older age, affected by frailty and/or complex long term health conditions and/or disabilities; often compounded by their mental health or emotional well-being needs.   The postholder will need to work closely with all the multi-professionals as well as GPs and Primary care and cluster staff, and the Third sector, to support individuals to have good quality emotional and mental health well-being.  They will be a key practitioner within the team developing and providing an effective targeted holistic team around the person.    

 

Main duties of the job

The post-holder will:

To provide initial screening assessment to identify Mental Health needs, provide short term early interventions and signpost as and when necessary to, third sector, self-help Tier 0 community services or appropriate mental health services as required.

The post holder will liaise closely with Local Primary Mental Health Support Services (LPMHSS), Secondary Care Mental Health Services, Older Persons Mental Health Services (OPMHS), Crisis services if required and Third Sector Organisations to ensure cohesive and purposeful pathways of care.

The post holder is identified as a skilled and creative mental health practitioner with knowledge of working with and supporting people living within their own communities and will be an integral member of the Integrated Community Network Teams providing a service for age 18 years up.

Provide short term mental health support to promote/empower independence, working closely with the Network MDT,. This will generally involve working with adults to promote positive mental wellbeing and to assess the need and suitability for mental health services covered by the Mental Health Measure (MHM).

 Complete risk assessments/management plans and escalate any concerns as appropriate.

Manage a defined caseload,.
