Job overview
An exciting opportunity has arisen to recruit an additional band 7 Nurse to our existing team of advanced care planning facilitators within CTM UHB.
Under the service group of palliative care - the post holder will have a key role in the co-ordination, facilitation and delivery of Advance Care Planning within Cwm Taf Morgannwg University Health Board for those individuals with and without mental capacity to support clinical decision making at end of life.
The post holder will be pivotal in the continued development of processes to support and embed Advance Care Planning across primary care, the independent sector and wider services.
The post holder will possess excellent communication and leadership skills and demonstrate an ability to work autonomously as well as an effective member of the team. The post holder will act as a specialist resource for other health and social care professionals on issues relating to advance care planning
.
Main duties of the job
· Promote the benefits and uptake of Advance Care Planning (ACP) with patients and their families in partnership with the Primary Care Team.
· Work as part of a multi-disciplinary team, fostering good communication with colleagues of all disciplines, whilst ensuring that all communications are conducted with dignity and respect.
· Liaise with Care Home Managers, District Nurses, General Practitioners and Out of Hours medical services to inform and advise on Advance Care Planning (ACP).
· Support teams and/or key health professionals in rolling out the principles of ACP across Cwm Taf UHB in collaboration with the Primary Care Team.
Working as part of a team the candidate will be expected to work across all localities according to service need including Bridgend , Merthyr and Rhondda localities. A car driver and access to a vehicle is essential for this role .
