Job overview
An exciting opportunity has arisen for and 8a Lead Advanced Clinical Practioner (ACP) to join a well-established Trauma and Orthopaedic Team as an Acute Frailty ANP at the Royal Glamorgan Hospital Llantrisant. The post holder will need to be in receipt of a MSc in Advanced Clinical Practice or equivalent qualification as well as an independent prescribing qualification.
The post holder will be expected to demonstrate advanced clinical practice skills and knowledge to enable autonomous assessment, management, investigations and diagnostic evaluations of the frail older patient. A clear understanding of frailty syndromes being essential to this position.
The post holder will need to demonstrate the ability to lead broad services and service redesign and change. Alongside this they will need to be able to demonstrate the ability to provide education and mentorship to a wide MDT audience. The post holder will need to carry out relevant research and audit and have a clear understanding of appropriate research methodology to enable evaluation of service changes.
Lead Advanced Clinical Practitioner to:
Lead a team of clinical practitioners in the context or orthogeriatric.
Leading on Comprehensive Geriatric Assessment in the context of pre and post operative care.
Service development
Research in relation to frailty and orthogeriatric care
Main duties of the job
Assessment and diagnosis - admit and discharge
Investigational management - therapeutic and non-therapeutic
Employ higher level communication skills to other health care professionals, patients and relatives
Carry out research in relation to frailty/ COTE
Manage the team and service on a day to day basis
Involvement and lead on HB strategy groups
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
