Job overview

Are you looking to work with service users in their own homes and community delivering Occupational Therapy and Physiotherapy led interventions? 

An exciting opportunity has arisen in the @Home service a Band 4 Generic Therapy Assistant Practitioner.

This is an excellent opportunity for somebody who has previous experience as a therapy or rehabilitation assistant in an NHS or care setting.

You will become part of the @ Home Team based in Dewi Sant Health park  which offers clinic based and home based assessment and interventions aimed at admission avoidance for the frailer older adult approaching crisis operating across the Merthyr and Rhondda Cynon Taf area. 

The GTAP  will work closely with the @Home Therapy staff, Consultant Geriatricians, Advanced Nurse Practitioners and  Nursing colleagues to complete delegated tasks and explore interventions to enable patients to remain at home. 

This will require positive risk reviews, adaption of the home environment,  implementing therapy interventions and liaising with community teams for onward rehabilitation and larger scale environmental adaptation.

Ideally you will have proven experience in a community setting.  Consideration will be given to candidates who have had community experience as part of their training or work experience.

Full support, training and CPD opportunities will be provided for the successful candidate to develop skills and competencies and to assist with implementing changes to practice and service delivery.

Main duties of the job

Supporting the  Occupational Therapists (OT) and Physiotherapists (PT) in the delivery of OT and PT management and rehabilitation to community patients/clients .

To work as part of a community based MDT to undertake specific clinical and administrative tasks, implement intervention programmes within the service user's home and community as instructed and delegated by the OT and PT, responding flexibly to the needs of the service.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Support the Occupational Therapist and Physiotherapist  by assisting with all modalities of therapeutic care for intermediate care and reablement patients.

Support the Occupational Therapist and Physiotherapist by providing supervision and assessment of support worker competencies.

To provide care to the patients, as delegated by the Occupational Therapist and Physiotherapist, to the extent of own competencies.

Participate in project work, audits and provision of relevant statistics which may be for the benefit of patients or the Occupational Therapist and Physiotherapist and @Home Service.

To promote independence, health and well-being and safety within the home and local environments. 

Develop a patient centred approach to engaging patients in routine therapeutic occupations following the aims/methods given by the Therapist and to adjust own clinical interventions based on the fluctuating needs of the person.

Qualifications and knowledge

Essential criteria
  • Evidence of good general education, English/Mathematics GCSE or equivalent qualification grade C or above.
  • NVQ level 3 in relevant subject or the equivalent level of knowledge and experience plus additional competency based training.
  • Detailed knowledge of rehabilitation intervention.
  • Comprehensive understanding of medical, orthopaedic, surgical and social related issues.
Desirable criteria
  • Expected to achieve competency based training within a two year period.

Experience

Essential criteria
  • Experience of working as an NHS band 3 OT, PT or generic assistant or equivalent non-NHS functional assessment/ rehabilitation role
  • Experience of developing/delivering education advice to groups.
  • Experience of team working.
  • Experience of working with the public.
Desirable criteria
  • Experience of community working
  • Experience of working with healthcare and/or LA professionals.
  • Previous experience within the NHS or LA with patients who have a range of physical, psychological, emotional and/or social needs.
  • Experience of supervising others.
