Job overview
We are looking for an experienced and enthusiastic Registered Nurse to join our Parkinsons nursing service within Primary Care and Community.
The post holder will be required to work across a range of care settings including Community Hospitals, Care Homes, GP Practices, and within patients’ homes as directed by the Senior Nurse.
The post holder will act as an expert resource providing specialist advice and support regarding the clinical management of patients presenting with Parkinsons in the community setting.
The post holder is identified as a skilled and creative practitioner with knowledge of working with and supporting people. We aim to provide a team around the person, which enables individuals to have the right support at the right time.
Acting within their professional boundaries, the post holder will provide support and intervention for the presenting person through holistic person centred assessment, clinical consideration, diagnosis, treatment and evaluation of their care within a multi-agency team approach. They will demonstrate safe, clinical decision-making and expert care and support for people.
Main duties of the job
Undertake skilled clinical assessment to deliver safe and effective interventions, advice and support for people with Parkinson’s disease and their families/carers. To manage a caseload of patients with PD ( Parkinson’s Disease), through the four stages (diagnosis, maintenance, complex and palliative), using evidence-based principles to assess individual needs, set and implement programmes of care, evaluate interventions in both the hospital and community setting. To participate in planning, developing and evaluation of PD services within the Health Board. Contributes to research based nursing practice and monitor the quality of the service provided. Undertake professional Activity e.g. Audit, CPD, teaching, research and administration. To deliver an education/teaching package regarding the management of PD to all health and social care staff in both primary and secondary care as well as private and voluntary organisations. |

Qualification and Knowledge
Essential criteria
- Registered General Nurse adult or dual registration mental health
- Current NMC registration
- Masters degree or equivalent experience specialist qualifications
- Expert level of knowledge in clinical specialty
- Evidence of CPD
Desirable criteria
- Teaching qualification
- Knowledge and interest in Parkinsons
- Non medical prescriber or to undertake
Skills and Attributes
Essential criteria
- Excellent written and verbal communication
- Ability to self manage and prioritise workload
Desirable criteria
- Evidence of ability to influence practice through the use of evidence base practice.
