Job overview

Main duties of the job

A vacancy have arisen with the Complex Care Team for experienced Registered Nurses (RGN, RMN or RNLD) to undertake the role of Complex Care Nurse Assessor, based in Cwmbran.

The role will require the post-holders to undertake in-depth expert / specialist nursing assessments of individual residents in nursing homes and hospital sites across the five Locality areas of Gwent, on behalf of Aneurin Bevan University Health Board, to ascertain individual’s eligibility for Continuing NHS Healthcare (CHC), Discharge to Assess and entitlement to NHS Funded Nursing Care (FNC) funding. Consequently, the roles will require the post-holder to facilitate the Multi-disciplinary Team assessment process, and to monitor the individual's changing needs and agreed packages of care within the homes.

The post-holders will promote collaborative and inter-agency working across professional and organisational boundaries by providing professional advice, support and guidance to both professional and non-professional colleagues, individual residents and their relatives / representatives

Recent NHS or Care Home experience  is essential.

A proven track record of being reliable and dependable.

A flexible approach to work would be a great advantage.

Essential criteria
  • Degree level’ education
  • Adult Safeguarding level 1&2
Desirable criteria
  • Post graduate certificate or degree in Community care / nursing or relevant area of nursing.
  • Short courses attended or postgraduate certificates in managing challenging behaviour, dementia

Experience

Essential criteria
  • Experience of undertaking needs based assessments and care plans
  • Undertaking capacity assessments, gaining formal consent and the application of DoLS and the Mental Health Act.
Desirable criteria
  • Community nursing
  • Knowledge of the Mental Health and Mental Capacity Acts

Knowledge

  • Dealing with cases with complex presentations
