Job overview

To support the Children’s Community Nursing Service in providing respite care, supporting outpatients clinics and special needs schools and some aspects of clinical care under the guidance of the registered nurse.

To work as part of the Children’s Community Nursing Team by supporting the registered nurse in delivering a high standard of care to the child/ young person and their family.

To work as part of the nursing and multi-disciplinary team in the child's own home and other establishments under the direction of a Community Children's Nursing Borough Manager.

Each team has a Team Manager, two Deputy Managers and a team of registered and non registered staff working across the boroughs.  The service is managed by a Senior Nurse and personal assistant based in Risca Health Centre .  Along with the Children’s Continuing Care Manager who has overall responsibility for the management of the children and young people's continuing care process within the community children's nursing service

 

Main duties of the job

An opportunity has arisen for enthusiastic HCSWs to join the Childrens Community Nursing Service (CCNS)  in ABUHB (CCN North and South Teams). We are looking for motivated individuals who enjoy working with children in a variety of settings and environments. The individuals must have the pre requisite Level 3 qualification in health and social care or childcare. 

Their role will be to support the CCNS in providing respite care, supporting outpatient clinics and special needs schools and some aspects of clinical care under the guidance of the registered nurse. 

To work as part of the CCNS by supporting the registered nurse in delivering a high standard of care to the child / young person and their family. 

To work as part of the nursing and multi-disciplinary team in the child’s own home and other establishments under the direction of a CCNS Team Leads. 

All necessary training will be given and the successful candidates will be supported to develop their knowledge, experience and skills. We also encourage wider development and career progression is encouraged. 

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Continuing Care 

The team are responsible for providing packages of care to children following a recommendation of hours approved by the funding panel. 

 
Care Closer to Home 

The CCN team provide a clinical service, the aim of the service is to provide a skilled nursing resource for children, young people and their families, reducing hospital admissions where possible and facilitating early discharge. The team accept a number of referrals from hospitals and community agencies. 

 Paediatric Outpatient 


The CCN team is also responsible for managing and staffing all paediatric outpatient clinics across ABUHB.

There are four special needs schools within Gwent supported by the CCN team

Experience

Essential criteria
  • Level III qualification in health and social care or childcare.
  • Previous experience in healthcare
Desirable criteria
  • Previous experience working with children/ young people and children with complex healthcare needs.

Knowledge and skills

Essential criteria
  • Ability to carry out instructions accurately and efficiently.
  • Excellent communications skills.
  • Knowledge and understanding of the need for confidentiality
  • Ability to work as part of a team or well as a lone worker.
  • Sound knowledge of safeguarding children and young people
  • Evidence of continued educational development.
Desirable criteria
  • Computer skills
  • Phlebotomy skills
  • Knowledge of Healthcare Support Workers Code of Conduct.
