Job overview

The district nursing service is fundamental in the delivery of the health boards’ clinical futures model in providing care closer to home enabling early discharge and preventing hospital admissions.  

We deliver holistic care to patients within their own homes and residential homes which meets the increasing complexities in health of our patient population. Assessment, planning and delivering high standards of care are achieved through effective collaboration and partnership working within our health and social care integrated centre at Rhymney. The centre houses Redwood Community Hospital, social services, the district nursing team, GP surgeries and many other health and social care professionals, thus, enabling a collaborative way of working and delivery of effective and timely care to patients of the community. 

Main duties of the job

To care for patients and their families within their own homes is a privilege and the Rhymney district nursing team have an exciting opportunity for a community staff nurse to join their supportive, dedicated and forward-thinking team. You will have the opportunity to develop your clinical and interpersonal skills with continuing professional development opportunities to prepare you with the skills to meet the evolving autonomous role of the community nurse through engagement of a robust induction programme.
