Senior Lead, Place Transformation (special projects)

NHS Norfolk and Waveney Integrated Care Board (ICB) plans and buys healthcare services for our local population. We are accountable for the performance and finances of the NHS across Norfolk and Waveney – a total budget of £2 billion a year. Known as NHS Norfolk and Waveney, the organisation works with local people, health and care professionals, and partner organisations to improve the health and wellbeing of our population.

The organisation is part of the Norfolk and Waveney Integrated Care System. A system dedicated to working with partners in local government, the voluntary sector and others and helping the NHS to support broader social and economic development and to tackle inequalities in health outcomes.

Our main offices are based at 8th Floor of Norfolk County Hall, Martineau Lane, Norwich, NR1 2UE however we have a local office for the East of the region based in Beccles and another in the West of the region based in King’s Lynn, therefore travel across the Norfolk and Waveney footprint as well as out of county is required according to where our stakeholders are situated.

Great Yarmouth and Waveney Place team has a 6-month secondment role, to support delivery of the local place agenda. As a Senior Lead Place Transformation and Delivery (Special Projects), you will work closely with others to bring about transformational change for local services and people, underpinned by local Primary Care partnerships.

You will be joining an established and supportive team, who have ambitious plans and a strong commitment to staff development and wellbeing.

This senior role will help to lead and shape ways of working with partners and will seek out opportunities to support the ambitions described in the N&W Joint forward Plan. The role will have a particular focus on partnering with our local primary care colleagues, to enable a positive impact on population health. By shifting our focus to preventative approaches and with the use of PHM insights, we hope to address people’s health needs more effectively, enabling individuals to live well.

You must have considerable knowledge and expertise of integrated working within the health and care sector in particular within primary care and have experience in leading complex work programmes. This role will manage a small team and include opportunity to work collaboratively with colleagues across community & secondary care, mental health providers, local authority and VCSE organisations.

Health and care services in Norfolk and Waveney are working closely together to further improve services and provide more joined-up care for local people. In Norfolk and Waveney, we have already achieved a lot by working in partnership to improve health and care outcomes. These changes have been made possible by different organisations – NHS hospitals, GPs, mental health and community health services, local councils, care homes and social workers, voluntary and community organisations and others – joining forces to agree and plan for local people’s needs.

Norfolk and Waveney Integrated Care System (ICS) includes a statutory Integrated Care Partnership (ICP), and an Integrated Care Board (ICB) called NHS Norfolk and Waveney. This partnership and organisation dedicated to making sure that organisations work together for the benefit of our residents, staff and communities is an important step change, helping to create positive differences to local people and joining up health and social care. This is the culmination of many years of effort to build partnership working across the NHS, local authorities, the third sector and patient groups.

The post holder will: 

  • Support the transformation and delivery of health and care programmes across the Locality, taking a lead role to develop and deliver the range of national, ICS and locality strategies and priorities including, but not limited to, Primary Care Networks, Integrated Neighbourhood development, Ageing Well Programme, Urgent and Emergency Care, and local operational surge and winter plans across health and care. 

  • Support the Locality team to ensure that programmes are planned and managed effectively and take the lead in assisting in their successful delivery. 

  • Support the Locality team in driving change to support improved outcomes for the population whilst ensuring value for money in planning, commissioning and service provision across all stakeholders.   

  • To further develop transformation agendas across the Locality team, identifying and if appropriate addressing variation. 

  • Support Place partners in the implementation of national strategies for example the Long Term Plan, GP Forward View etc, ensuring implementation, adherence and monitoring. 

  • Develop relationships and forums to ensure close partner working across the Locality team, providing ongoing support, in particular within General Practice Provider Organisations, Clinical Directors, GP Practices, community and secondary care, social care and mental health.
