Suffolk CRHTT are now under new operational Leadership and line Management making it an exciting time to join the team. Are you an experienced mental health professional looking to utilise your clinical skills in a different setting? The CRHT teams are seeking Senior Crisis Care Practitioners to join us.

The Crisis Response and Home Treatment Team are looking to recruit compassionate, dedicated, and conscientious individuals who want to make a positive difference to adults experiencing mental distress and mental health crisis.  CHRT are looking for band 6 Practitioners who are positive change agents to lead a Transformation  agenda across the team.

This role is to work across the crisis support pathway based in Suffolk, predominantly Woodland (East Suffolk) or Wedgwood (West Suffolk Hospital) with vacancies within both teams. 

You will work within a multidisciplinary team providing care, treatment, and assessment to patients both at base and in their own homes.

Main duties of the job

The CRHT team are very proactive and highly skilled at managing risk and supporting service users in West Suffolk who present in crisis and persons needing intensive support to avoid hospital admission.

We are - the ‘front-door’ to acute services, providing triage, assessment, intensive support, and alternative to admission hand in hand with support of families and 3rd party providers.

The staffing teams also works with service users already in hospital to ensure that they can be supported at home at the earliest opportunity to support their recovery.

CRHT delivers patient care twenty-four hours a day. Shift working is in place to ensure this service is delivered.

There is an expectation that you will have the ability to clinically lead care delivery and will be proficient in clinical care including care/discharge planning, risk assessment, admission prevention, environmental health & safety, Mental Health Act work and physical health care.

We are looking for professionals who want to be involved in assessments and home treatment for people in crisis. CRHT is an age inclusive service.

Essential criteria
  • Nursing degree or equivalent, Dip OT, Clinical Psychology or equivalent
  • Ongoing professional development working towards degree level

Experience

Essential criteria
  • Able to demonstrate an appropriate level of knowledge of mental health practice from previous experience
  • Demonstrate effective risk assessment and risk management experience
  • Experience in delivery of clinical interventions
Desirable criteria
  • Experience in delivering group based interventions
