Following a promotion, we have an excellent opportunity for an experienced Midwife to join the senior leadership team as our  Quality and Governance Matron.

We have made significant progress on an improvement journey within Maternity Services, and have a new leadership team in place following substantive additional investment in leadership roles within the service.

This investment includes additional midwifery posts to support the Governance arrangements within the service, so this is an exciting time to join our team.

The Quality & Governance Matron will lead, co-ordinate and advise on all aspects of clinical quality, risk management and governance processes across maternity services. The team is well resourced, and the post holder will work as an integral member of the maternity leadership team to ensure we are embedding learning from incidents and patient feedback within the service as we continue our progress.

The post holder will be required to review, define, scope, plan, manage and deliver activities associated with the quality agenda for maternity.

We have developed an Integrated Performance Report which is a barometer for the service, and expect that further clear outcome measures will need to be established to ensure that systems and processes are in place to consistently meet local and national standards and policies.

The Quality and Governance Matron will lead, co-ordinate and advise on all aspects of clinical quality, risk management, governance processes in Maternity services.

The post holder will be required to review, define, scope, plan, manage and deliver activities associated with the quality agenda for Maternity and Midwifery services. Clear outcome measures will need to be established to ensure that systems and processes are in place to consistently meet local and national standards and policy. The post holder will lead on key initiatives such as NHS Resolution Maternity Safety Actions (CNST).

The post holder will be an excellent, visible midwifery leader and credible role model with expert clinical knowledge and experience and sound understanding of governance processes. The post holder will be instrumental in monitoring performance and delivery of excellence in clinical care.

 Duties will include leading investigations into serious incidents and potential litigation cases, ensuring timely investigations into all serious incidents and preparation of preliminary reports. The Post Holder will implement, monitor and develop the maternity clinical risk management strategy in line with the Trust’s clinical risk management strategy.

The post holder will lead the Maternity Risk and Governance team, including the Quality Improvement Midwife and the Bereavement team as well as provide leadership and support for healthcare governance activities within maternity services.

The post holder will work with the Head of Midwifery, Deputy Head of Midwifery, Consultant Midwife, the Maternity Quality & Workforce Matron and Associate Director of Improvement in Maternity to: 

  • Work with the Clinical Lead, Senior Maternity Management Team, obstetricians and  Maternity Voices Partnership to ensure a quality service that is responsive to the needs of service users
  • To ensure that the Maternity Risk and Governance Strategy is effectively conveyed to all staff and is translated into operational practice.
  • The post holder will undertake and support others investigating incident including the writing up of reports/learning responses of such investigations.
  • To conduct regular reviews of previously reported incidents and evaluate their action plans to determine whether or not recommendations and learning has been implemented.
  • To develop appropriate action plans to minimize risks, including identifying resource implications, changes in clinical practice and administrative procedures required.
  • The post holder will be responsible for developing a planned programme of work to maintain CNST safety standards within Maternity.  The post holder will be responsible for the collation of all relevant documentation, and will be expected to support relevant members of the Division of surgery in their contributions to ensuring that standards are both met and maintained.
  • Ensure there is a robust performance management framework within the service to deliver on all aspects of the risk management and clinical governance agenda.
  • To maintain the Maternity/Obstetric aspects of the Risk register – this involves working with clinical staff to identify risks and carry out risk assessments.
  • To monitor the Maternity risks and regularly update risk status as appropriate
  • To act as a clinical role model promoting professional, competent standards, working closely with the multi-disciplinary team.
  • To coordinate the Maternity Governance Committee, Maternity Incident Review and Escalation forum and Labour Ward Forum. 
  • To be highly visible and approachable to staff, women and their families using the service, and the public so that open and honest communication channels are created and effective assessments of care delivery and risk management can be undertaken.
  • To be contactable whilst on duty to provide advice and support to staff if required and support with clinical duties if professionally registered (within their scope of practice).
  • To undertake extended hour roles as appropriate to support the delivery of the quality agenda throughout the 24 hour care period.
  • Develop Clinical Risk Management initiatives and strategies
  • Work in collaboration with the Consultant Midwife to identify and action quality improvement initiatives.
  • Investigating incidents completely and thoroughly, using the appropriate methods in line with the Trust Patient Safety Incident Response Plan which are aligned with our Trust priorities and ambitions.
  • Liaise with the Trust wide and external stakeholders (CCG, LMNS, HSIB, MBRRACE, CDOP) to ensure Patient Safety Incident Response Investigations are conducted in line with national and local requirements. Ensure recommendations for all staff involved is communicated effectively instigating change in practice where necessary. Ensure any learning is shared with a system-wide learning approach where appropriate.
  • Providing statistics as required by the Division or the Trust including high level reports relevant to quality, risk and governance.
  • Assisting the Legal Department in the management of litigation cases, providing advice, support and written reports as required.
  • To manage and maintain the Maternity programme of audit and guideline management and, where changes in practice/service is required, ensure the required governance surrounds the change so that patient safety is maintained.
  • To be integral in the identification of external recommendations, perform GAP analysis and monitoring of those action plans to address any gaps identified.
  • To report to and work with MBRRACE, PMRT, HSIB and NHS Resolutions.
  • To be a UKOSS lead.
