Job overview

An exciting opportunity has arisen for an experienced and highly motivated Nurse to work as NHS Continuing Healthcare Nurse/Practitioner for Discharge to Assess Pathway 3.

 This post offers the opportunity to enhance assessment, decision-making skills and complex case management skills in a challenging but supportive environment. You will also be a member of the Integrated Discharge Hub. You will be responsible for assisting with the discharge planning, assessment and commissioning of care for individuals from the acute hospital.

We are looking for a registered nurse who is keen on integrated working, an understanding of hospital discharges would be desirable, but support and guidance will be on hand. You will be adaptable, have good communication skills and enjoy working with other professionals.  This post is based in the Norwich Community Hospital, working very closely with the integrated discharge hub but you will be undertaking assessments in the acute hospitals, which will involve traveling to visit patients and attend meetings. 

You will offer a person-centred approach to individuals and their families.  Working in partnership with other agencies and have a strong commitment to improving the outcomes for those on your caseload.  You will have proven skills in working with people who have complex health needs.  You will also be able to demonstrate that your practice is evidence based and innovative.

 

Main duties of the job

  • To jointly assess with other health and social care colleagues, patients ready for discharge from hospital to identify who will be accountable for case managing whilst on the Discharge to Assess Pathway.
  • The post holder will take responsibility for collaborative working practices to deliver the D2A3 (complex discharge) Pathway supported by the Band 8a Senior Nurse.
  • The post holder will work collaboratively with the acute hospital patient facing services to support successful discharge on a personalised patient pathway.
  • Where required will review with the Senior Nurse to establish an effective interface between D2A3 and existing hospital discharge pathways.
  • Ensure high quality, safe and timely patient flow from the hospital through to discharge within the community.
  • Ensure the accurate application and quality assurance of decision making, ensuring patients have access to the full range of available and appropriate discharge pathways.
  • Provide effective communication with patients and their families to ensure engagement and understanding of the D2A3 Pathway and potential onward transfer and support.

Detailed job description and main responsibilities

Proactively manage a delegated caseload and have the ability to function
autonomously. Demonstrate timely forward planning and an ability to organise self.

This includes three month/annual quality of care and eligibility reviews for
NHS Continuing Healthcare as per the National Framework when necessary.

Complete Care Plans within three months of eligibility for all patients in receipt of NHS Continuing healthcare or joint funding when necessary.

To complete relevant specific core reviews/assessments for NHS funded
individuals e.g. Deprivation of Liberty Safeguards/Liberty Protection Safeguards reviews of Clinically Assisted Nutrition and Hydration for individuals that lack capacity to consent to this

Ensure continuity of care by appropriate transfer of information to ensure high standards of care.

Be proactive in Quality Assurance and identifying deficits in patient care, liaising closely with associated Quality Improvement Nurses for Care Homes and Medicines Optimisation Team.

Participate in professional/multi-professional meetings and conferences as
required.

Monitor the standards of care received by patients receiving NHS Continuing Healthcare, NHS-funded Nursing Care and Personal Health Budgets alerting other stakeholders when appropriate.

Direct patients and next of kin/Power of Attorney to appropriate Advocacy
Support Services.

Provide specialist knowledge and expertise to ensure that equipment
requirements are met. To include the authorisation of equipment.

Escalate appropriately to relevant Lead Nurse for NHS Continuing Healthcare

Regular attendance at Primary Care Network meetings as these evolve across Norfolk and Waveney

Occasional travel outside of the Norfolk and Waveney area
