Paediatric Virtual Ward Co-ordinator

Job overview

To be a lead contact for the paediatric virtual ward at QEH in conjunction with the clinical team. 

To support the identification of patients who are suitable to be discharged out of hospital onto the virtual ward. 

To coordinate the remote monitoring and technological aspect of the virtual ward including parent education and support.

To liaise with all aspects of the MDT to ensure greater patient experience and that the virtual ward is run in a timely, effective, and efficient manner.

Main duties of the job

Contribute to effective communication within the multidisciplinary team to ensure the delivery of high standards of care in the virtual ward.

Attend board and ward rounds to raise the profile of referral into the virtual ward.

Liaise and work alongside the Virtual Ward MDT, regularly communicating with clinical teams and other allied health care professionals a to ensure identified needs are met and care co-ordinated appropriately.

Participate in virtual ward MDT meetings alongside the clinician responsible for the virtual ward to ensure communication and patient safety is maintained.

Following a holistic assessment, devise an individualised, evidence-based care programme for each patient on the virtual ward, modifying it as required.

Be responsible to deploy the remote monitoring equipment to the virtual ward patient, undertaking consent and parent/carer teaching as appropriate.

Ensure informed consent is obtained prior to initiating interventions. 

Where there are barriers to understanding, such as hearing impairment, mental capacity impairment and other difficulties in comprehension, explanations require adjustments in order to gain understanding, consent and concordance. 

With the virtual ward team, participate in the planning and delivery of the discharge process, ensuring that patients are discharged off the virtual ward in a timely, safe and appropriate manner.

  • Level 3 NVQ/Foundation degree in Health Care Studies or willingness to work towards

Experience

Essential criteria
  • Experience of working in an acute paediatric environment for more than 1 year
  • Teaching and leadership experience within ward
  • Be able to document to a level required by NMC standards
Desirable criteria
  • Knowledge of the wider health care setting
